NCDOT Division 5 proposes to replace Bridge No. 900052 over in Weaver Creek on SR 1518 (Stewart Farm Road) in
Vance County. Note that this is part of the State Funded Year 5 bridge program. The existing bridge is being replaced
with an aluminum box culvert. The following information is attached as part of this permit application:

1. Avoidance and Minimization
1a. Specifically describe measures taken to avoid or minimiie the proposed impacts in designing project.
The bridge is being replaced with a single barral Aluminum Box Cul�ert (ABC). The single barrel provides the sarne width as the existing br+dge and
spans the low flow channel without reducing the stream width. Tha proposed culvert provides more effective hyclraulio opening than the existing
bridge and will be replaoed in-place. The cul�ert will be buried to maintain stream bedform and there will be no excavation in non-permitted
jurisdictional areas. Promotion of sheet flow and infiltration over riplrap bank stabilization or grassed su►faoes is proposed.

The purpose of this letter is to notify you that the NCDEQ Division of Mitigation Services (NCDEQ DMS)
will provide the stream and riparian wetland mitigation for the subject project. Based on the information supplied
by you on March 16 and 28, 2016, the impacts are located in CU 03020101 of the Tar-Pamlico River basin in the
Central Piedmont (CP) Eco-Region, and are as follows:

This mitigation acceptance letter replaces the mitigation acceptance letter issued on March 22, 2016.
NCDEQ DMS commits to implementing sufficient compensatory stream and nparian wetland mitigation credits to
offset the impacts associated with this project as deternuned by the regulatory agencies in accordance with the In-
Lieu Fee Instrument dated July 28, 2010. If the above referenced impact amounts are revised, then this mitigation
acceptance letter will no longer be valid and a new mitigation acceptance letter will be required from NCDEQ DMS.
