Please provide the Required Items Checklist (second worksheet of the Level
Spreader Supplement Form) with all required items attached for each of the
proposed level spreaders. Because not all of the required items were included in
the December 21, 2007 submittal (particularly complete and accurate topographic
details of the filter strips), the DWQ is not able to adequately evaluate the
proposed level spreader designs at this time.
2. It appears that Level Spreader Number 1 has been proposed up slope of a draw. If
this is the case, please relocate this level spreader to a more suitable area.
