1. Please provide documentation of the City of Raleigh's approval of the stormwater
management plan for this project by providing EITHER:
A valid approval letter from the appropriate DWQ regional office or local
government indicating that the proposed activity has an approved SMP and
one copy of the approved SMP, including plan details, calculations and other
supporting information, OR
• A set of stormwater plan details and calculations stamped as "Approved" by
the City of Raleigh.
2. Please provide plans of the entire site at a scale of 1" = 50' or larger as requested
in Item 1 of the Required Items Checklist of the Level Spreader Supplement
Form.
3. Please provide a section view of the level spreader at a scale of 1" = 20' or larger
as requested in Item 3 of the Required Items Checklist of the Level Spreader
Supplement Form.
