Monthly monitoring is required for the Fayetteville facility as per the Tiered program requirements in Part II, Section B of the facility permit for FV -002. For the monthly sample period monitoring, a sample was taken on May 9, 2015 for FV -002. The analytical results with associated parameters for this semi-annual monitoring are included in the table below. I certify, under penalty of law, that this document and all attachments were prepared under my direction or supervision in accordance with a system designed to assure that qualified personnel properly gather and evaluate the information submitted.
