e. PCN Application Form Attachments (12 pages)
f. Attachment A — Preliminary Jurisdictional Determination Form (5 pages)
2. Fee Assessment (check for $240.00)
3. Full size project plans (2 sets)
4. 11 x 17 project plans (3 sets)
An Environmental Information Document has been included in the Engineering Report submitted to the
North Carolina Department of Environmental Quality, Division of Water Infrastructure for their review
and approval and issuance of the correct final environmental document, either a Categorical Exclusion
(CE) or Finding of No Significant Impact (FONSI).
