On behalf of the applicant, the Town of Apex Public Works & Utilities Department, please find attached a complete
application and supplemental information requesting an updated Nationwide Permit 12 verification from the U.S.
Army Corps of Engineers (USACE) and an updated Water Quality Certification 3884 from the N. C. Division of Water
Resources (NCDWR). This After -the -Fact modification request is the result of unanticipated construction activities
consisting of dewatering, sand bags and debris removal. In addition, the applicant would like to request an updated
Jordan Lake Watershed Riparian Buffer Authorization from the NCDWR for the additional impacts to riparian buffers.
