A check for the permit fee in the amount of$1,000.00. 2. Two Copies of the revised application. 3. Supplement-EZ Form. 4. Signed, sealed and dated drainage calculations. 5. Copy of the soils report. Please note that this report was not submitted earlier because it was done for foundation design of the buildings and not for stormwater design.As noted in the design narrative, a separate test hole was done in the stormwater basin for determining the water table elevation.Soil composition in this location was not done because the basin design called for replacement of existing soils with the specified, high permeability soils material.The soils report that is attached does measure the water table elevations in other areas of the site and the depths to the water table are consistent with the one taken in the basin. 6. As-Built Permittee Certification,As-Built Designer's Certification for General MDC's and As-Built Designer's Certification for Sand Filter Project. Thank you for your assistance.
