I just resubmitted both of these on the portal. I saw where the date was incorrect for March GW59 and corrected that before I submitted. I hope everything is correct now. Per our phone call, I am following up with an email about still outstanding data concerning permit WQ0004910. As of today, we are still missing two vital reports for your facility’s permit requirements. Specifically: 2 1. March 2024 GW-59 reports – Based on my records you originally submitted these forms on 10/22/2024. They were reviewed on the same day by Wanda Gerald in our central office and rejected due to missing and/or incorrect month/year. You resubmitted the forms on 10/28/2024 and they were reviewed and rejected again on 10/29/2024 for the same reason. I am unsure which date was incorrect or missing. But there are only three dates on the form that need to be correct. (See locations circled in red on the example form below). a. Expiration date: This is found in the far upper right-hand part of the form. This is the expiration date of your permit, which for you is 10/31/2027. b. Date sample collected: This date is when the groundwater samples were collected from the well by Waypoint Environmental. c. Signature date: Located at the bottom right corner. This is the date the forms were signed by Mr. Eaton. 2. June 2024 NDMR and NDAR-1 forms have not been received. It appears that you submitted them on 10/21/2024. They were reviewed on 10/22/2024 and were rejected because the submittal also included the June GW-59 forms. These need to be two separate submittals, one for GW-59s and one for NDMR/NDAR-1 forms. We have received and accepted the June GW-59 forms. Please resubmit the June 2024 NDMR and NDAR-1 forms. Please submit this data as soon as possible. We need the missing information submitted no later than close of business on Wednesday, November 20, 2024. Failure to do so, will result in enforcement against the Town of Woodland and Mayor Beale including, but not limited to a Civil Penalty Assessment and fines.
