Two Sets of sealed Plans @ 24" X 36" (signed and sealed) 2. NCDEQ SWU Wet ink Application 3. NCDEQ Wet ink O&M Application 4. NCDEQ EZ Form In addition, please refer to the following responses to comments: 1. Comment: Section IV, 10 - Low density projects should use only 1 drainage area for the whole property indicating any existing BUA that will remain along with the proposed additional. Response: Drainage 1 and Drainage 2 has been combined as per email conversation with Mr. Lawyer. 2. Comment: The second page of the provided Supplement-EZ Form does not contain the remainder of the form, which provides the full Drainage Area breakdown table. You must click where it says CLICK TO LOAD FORM' to get the remainder of the form/page Response: Drainage area portion of Page 2 is loaded and filled out. 3. Comment: 1x original, signed & notarized hard copy of the complete O&M Agreement Form (This item is required per 15A NCAC 02H .1042(2)(j) and per Sections V & VI, 3 of the Application). This form can be found on our website at the following address: https://deq.nc.gov/about/divisions/energy-mineral-and-land- resources/stormwater/stormwater-program/stormwater-design. Please note that a complete O&M Agreement Form will have 2 or more pages and uses macros to generate the relevant sheets based on the information entered into the cover page. NOTE: It is recommended to get a second copy of this form signed & notarized for the Applicant's records (it will be helpful when they go to record it with the Register of Deeds). An O&M Agreement Form was not included with the submittal. Response: The NCDEQ O&M document is filled out and provided. A second wet ink form has been prepared as per the recommendation. 4. Comment: Wetland delineation or a note on the plans stating that there are no wetlands located on-site. This item is required per 15A NCAC 02H .1042(2)(g)(iii) & Section VI, 8m of the Application. Please also list the name of the individual that made this determination in a note on the plans Response: Upon site visit, there are no wetlands present, and a note of the field inspection has been added to the bottom right of C200, C300, and all three DA sheets. DA-DEQ has been removed due to obsoleteness, this sheet was provided to ensure DEQ was aware of item 10's breakdown but is no longer needed. 5. Comment: While not required to be considered administratively complete, please note that Low Density projects require runoff to be dispersed through vegetated areas or conveyed through vegetated conveyances if dispersed flow cannot be achieved. Conveyance through rip-rap or pipes is not allowed. Response: The drop inlet is removed and converted into a culvert that discharges north which then disperses over vegetation before the property setback Please contact me if you have any questions or concerns.
