If using a permittee responsible mitigation plan, provide a description of the proposed mitigation plan. The Town ofClayton hired S&EC to delineate all potentially jurisdictional wetlands, streams, waters and buffers within the work area.S&EC verified our findings with both the USACE and DWR. S&EC engineers worked with the Town on the plan to minimize stream loss and stabilization and maximize the use of natural channel design elements (boulder weirs, constructed riffles, etc.). Stream piping was limited only to the most severe areas of stream erosion and within areas with immediate hazards to existing properties, homes and sanitary sewer lines. All required land disturbance and erosion and sediment control permits will be obtained prior to construction. The limits of disturbance area and removal of existing vegetation will be minimized to the greatest extent possible. All disturbed areas will be stabilized with erosion control matting and/ or ground cover as needed to help protect water quality during construction. Construction drawings have been included within this application showing these protective measures. Choose one Choose one Choose one Choose one Page 7 of 10 6. Buffer Mitigation (State Regulated Riparian Buffer Rules) – required by DWQ 6a. Will the project result in an impact within a protected riparian buffer that requires buffer mitigation? Yes No 6b. Comments: S&EC met on site with Ms.Cherri Smith (DWR)who confirmed the severe stream erosion, loss of property, and potential hazard to existing homes and the Town's sanitary sewer lines. Ms. Smith confirmed that the proposed work would fall under the "allowable" use of "Protection of existing structures, facilities and streambanks when this requires additional disturbance of the riparian buffer or the stream channel." As such, no buffer mitigation is required. Page 8 of 10 PCN Form – Version 1.4 January 2009 E. Stormwater Management and Diffuse Flow Plan (required by DWQ) 1. Diffuse Flow Plan 1a.
