EMERGENCY SPILL RESPONSE PLAN
BASIS: Major discharges of wastewater cause vast amounts of damage to the
environment, and place human life at risk. This plan is intended to focus on prevention,
control and should an event occur, countermeasures (mitigation) by identifying potential
hazards, evaluating engineering controls, work practices, administrative controls, medical
management, training, disposal, and establishing appropriate procedures.
GENERAL: The primary purpose of this plan is the prevention of a discharge as opposed
to "after-the-fact" or"reactive" clean-up measures. The threat of a major wastewater spill
poses serious problems to this employer. This plan establishes provisions to minimize
the hazards to human health or the environment when unplanned discharges reach the
soil, surface water, or groundwater. It is designed to ensure that the affects of potential
discharges are evaluated, prevention measures implemented, and if necessary,
remediation actions carried out.
PROGRAM RESPONSIBILITY: The Utility Systems Superintendent/Construction
Supervisor/Operator in Responsible Charge or Backup Operator in Responsible Charge
are responsible for all facets of this program and have full authority to make necessary
decisions to ensure success of the program.
SPECIFIC DUTIES: Specific duties of the Utility Systems Superintendent/Construction
Supervisor/Operator in Responsible Charge and Backup Operator in Responsible Charge
or his designee include, but are not necessarily limited to the following:
1. Coordinating program requirements with administrators and other employees
to develop and implement appropriate contingency and spill prevention, and
control policies and practices.
2. Monitoring procurement and use of spill equipment, including determining that
facilities and training levels are adequate for the procedures in use.
3. Performing regular formal inspections including inspections of emergency
equipment.
4. Maintaining current knowledge concerning the legal requirements of this plan.
5. Reviewing and implementing improvements to the Plan on an annual basis.
6. Maintaining overall responsibility for the Plan.

CITY OF ELIZABETH CITY WATER POLLUTION CONTROL SYSTEM ESR PLAN
1.0 INTRODUCTION. The information contained herein details the provisions made to
minimize the hazards to human health or the environment from any unplanned sudden or
non-sudden release of wastewater to soil, surface waters, or ground waters. This plan is
to be invoked by the spill response coordinators should there be an imminent or actual
emergency situation involving the aforementioned events.
1.1 THE PLAN. These response provisions satisfy the in-house regulatory
requirements of the City of Elizabeth City as per OSHA Standards and State
requirements will hereafter be referred to as the "plan".
1.1.1 The plan further addresses release reporting and related information
as per House Bill 1160, the Clean Water Act of 1999.
1.2 REVISIONS TO THE PLAN. The plan will be reviewed annually, and
immediately amended, if necessary, whenever:
1.2.1 Applicable regulations are revised.
1.2.2 The plan fails in an emergency.
1.2.3 The list of emergency coordinators changes.
1.2.4 The list of emergency equipment changes.
1.2.5 Required by the State of North Carolina, Division of Water Quality
after a review of the plan, submitted because of an incident.
1.2.6 Fifty Eight (59) sheets for pump station emergency procedures that
require changes.
These revisions are to be amended to the plan, and distributed as necessary.

2.0 EMERGENCY RESPONSE
This plan details the guidelines to be followed in the event of a non-routine occurrence
such as a release of wastewater into surface waters, soil, or ground waters, which poses
or potentially poses a threat to human health and/or the environment.
The (ORC) Operator in Responsible Charge/Backup Operator in Responsible Charge
and/or Spill Response Coordinators on site shall keep the City Manager (or his designee)
informed of the situation during the emergency.
2.1 EMPLOYEE SPILL PROCEDURES
(a) SPILL DETECTION ACTIONS. Anyone detecting a spill, shall:
• Notify their supervisor immediately
• Notify (ORC) Operator in Responsible Charge or Backup
Operator in Responsible Charge IMMEDIATELY
• Assess and evaluate spill
• Notify response team
• Attempt to stop or contain spill safely
• In the event the spill is estimated greater than 1000 gallons
— Notify City Manager's office to initiate press release
• Notify the State Emergency Operations Center within 24
hours — 1-800-858-0368 / 1-800-662-7956.
If the amount of the release is known; the following categories apply:
0 Minor Spill: A minor spill is one that involves the release of less
than 1000 gallons that does not reach surface waters.
0 Major Spill: A major spill is one that involves the release of
greater than 1000 gallons that reaches surface waters.
(b) SPILL CONTROL ACTIONS. To prevent further pollution, the
spill source shall be determined and additional spillage curtailed, if
possible, and it is safe to do so. In no case should any person
attempt to stop a spill without proper equipment or personnel backup.

SPILL NOTIFICATION ACTIONS:
• A press release must be issued after a discharge to surface
waters of 1,000 gallons within 48 hours of first knowledge of
the spill by the owner/operator. The law requires that the press
release must be issued to "all electronic and print news media
outlets that provide general coverage in the county where the
discharge occurred." A copy of the press release must be
maintained for one year by the owner/operator. This press
release is required in addition to the permit requirement of
contacting the North Carolina Division of Water Quality
(DWQ). See Appendix for model press release and list of
media outlets.
• If a discharge of 15,000 gallons or more reaches surface
water, a public notice is required in addition to the press
release. The public notice must be placed in a newspaper
having general circulation in the county in which the discharge
occurred and the county immediately downstream. See
Appendix for sample public notice format.
• If a discharge of 1,000,000 gallons of wastewater or more
reaches surface waters, the DWQ regional office must be
contacted to determine in what additional counties, if any, a
public notice must be published. See Appendix for sample
public notice format.
A copy of all public notices and proof of publication must be sent to the DWQ to
the attention of "NCCEU" at the letterhead address within 30 days of publication.
2.2 Duties of (ORC) Operator in Responsible Charge/Back Operator in
Responsible Charge. Responsibilities shall be carried out IAW North Carolina
Administrative Code Title 15A, Chapter 8G and North Carolina General Statutes
Chapter 90A-42 as outlined in the regulations governing certified Water Pollution
Control System Operators and any policies or programs in place by the City of
Elizabeth City, North Carolina.

2.3 DUTIES OF SPILL RESPONSE COORDINATORS. The spill response
coordinators has been granted the authority to execute this plan and utilize any
resources contained within this plan to minimize the hazards to human health or
the environment from any unplanned sudden or non-sudden release of wastewater
to soil, surface waters, or ground waters. All spill response coordinators will be
state certified and hold a valid water pollution control system operator certification
grade 1 or higher. Once an incident has been reported to DWQ Emergency
Operations Center (1-800-858-0368), the spill response coordinator is to report the
incident to the (ORC) Operator in Responsible Charge or Backup Operator in
Responsible Charge immediately, the (ORC) Operator in Responsible Charge or
Backup Operator in Responsible Charge and/or spill response coordinator is to
investigate the discharge as follows:
0 Identify the type of discharge
0 Identify the source of the discharge (e.g. what is causing the discharge).
0 Attempt to visually determine if the discharge represents a threat to human
health or the environment.
0 Attempt to determine the extent of the discharge (e.g. to what degree is the
discharge effecting the soil, surface waters, or ground waters as well as any
property damage).

2.4 DISCHARGE REPORTING REQUIREMENTS. Once these items have
been established, the (ORC) Operator in Responsible Charge/Backup
Operator in Responsible Charge and/or Spill Response Coordinators must
make a determination as to whether the site and/or surrounding area should
be secured and to what extent. The (ORC) Operator in Responsible
Charge/Backup Operator in Responsible Charge and/or Spill Response
Coordinators also has other individuals, who serve as resource personnel
to assist him/her and may be summoned upon request to ensure the
security and safety of the site to employees, citizens and the environment.
The phone numbers for these individuals are listed in Section, Other
Emergency Response Personnel. If the event involves a release, such as
a discharge, it may require reporting to the state agency.
The City Manager's office shall be contacted within twenty-four (24)
hours of first knowledge of the spill in the event of a discharge
requiring a press release. In the event the City Manager's office is
unavailable, the Assistant City Manager shall be contacted to activate
the press release, again within the same twenty-four (24) hours of first
knowledge of the spill.
A written summary of the incident will be completed by the site supervisor
within (5) days of the discharge and submitted to the Division of Water
Quality (DWQ) should the discharge be reportable. If the discharge involves
a spill, the (ORC) Operator in Responsible Charge or Backup Operator in
Responsible Charge and/or spill response coordinator will determine if the
spill/leak is a major or minor based on the following definition:
0 Minor Spill: A minor spill is one that involves the release of less than
1000 gallons that does not reach surface waters.
0 Major Spill: A major spill is one that involves the release of 1000 gallons
or more that does reach surface waters.
• Non-reportable: A non-reportable spill is less than 1000 gallons that

signed by the (ORC) Operator in Responsible Charge or the Backup

sewer overflow/spill.
• Reportable: A reportable spill is any amount of untreated sewer that
reaches surface waters/storm drainage system, soil or ground waters
which poses or potentially poses a threat to human health and/or the
environment and must be visited by the Operator in Responsible charge
(ORC) or Backup Operator in Responsible Charge (BUORC) within 24
hours of spill.
If the event involves a major spill, the City Manager's office shall
be contacted within twenty-four(24) hours of first knowledge of the
spill to activate the press release. In the event the City Manager's
office is unavailable, the Assistant City Manager shall be contacted
to activate the press release, again within the same twenty-four(24)
hours of first knowledge of the spill.
In the event sample collection is required, notify appropriate personnel
Or Section 2.6 other response personnel to collect samples from
nearest receiving water of discharge. Two samples each must be
Collected downstream and upstream of receiving waters at 50 and 100
foot intervals. All samples shall be taken to the Wastewater Treatment
Facility Lab for analysis. The Lab at the Wastewater Treatment
Facility shall maintain enough sample bottles and lab supplies to
perform analysis for three spills at one time to support the Spill
Response Program. The samples will be placed in a high priority
Status and shall be prepared as soon as delivered and read in 24
hours plus or minus 2 hours with the results faxed to the Director of
Public Works/Chief Superintendent/Operator in Responsible Charge
and/or Backup Operator in Responsible Charge. The 14'Jon Boat shall
be maintained in operational status at all times in case of an
emergency or to collect samples.
The (ORC) Operator in Responsible Charge/Backup Operator in
Responsible Charge and/or Spill Response Coordinators must complete all
applicable items contained in the Appendix and/or submit a summary
discharge report to the Division of Water Quality should a major release
occur.
The Emergency Response Team will ensure that the security and safety of
the area is maintained during the discharge, and while assistance is being
deployed. The Emergency Response Team will also ensure the security

and safety of the area is maintained during remediation, restoration of
equipment, and decontamination.

3.0 REMEDIATION.
3.1 Once the (ORC) Operator in Responsible Charge/Backup Operator in
Responsible Charge and/or Spill Response Coordinator and Emergency
Response Team determines the area is safe from any further threat, remediation
may be implemented. The spill response coordinator or designee must be on site
to ensure all remediation activities are properly managed.
3.2 All material including soil and water contaminated with discharge must be
removed and put into containers for disposal if applicable. The emergency
response team for ultimate disposal will characterize the material.
4.0 RESTORATION OF EQUIPMENT. Restoration of all emergency equipment used
during the discharge shall begin immediately following the emergency discharge.
Shovels, rakes, and other reusable equipment shall be consolidated into the least number
of drums possible, labeled, and placed in proper storage for decontamination. All process
equipment exposed to hazardous waste shall be cleaned using an appropriate solvent.
The used cleaning solvent shall be labeled appropriately and placed in proper storage.
5.0 DECONTAMINATION. All personnel exposed to wastewater discharges are to be
decontaminated as appropriate. This may involve washing adequately with soap and
water or a nontoxic cleanser, which will not harm the individual.
6.0 PLAN AUTHORIZATION. This plan has the support of management personnel. It is
understood that in the event of an emergency as described herein, the plan may be
implemented at the discretion of the spill response coordinator and/or designee.
7.0 TRAINING. Employees involved with the handling of wastewater discharges or spills
are trained on an annual basis. New employees shall receive training within one (1)
month of their date of employment. Emergency responder personnel shall receive
training semi-annually. All employees that hold State certifications on the collection
system are responsible to maintain their license in good standing.

ELIZABETH CITY, NORTH CAROLINA OPPORTUNITY TO PREVENT SSO's
GOAL STATEMENT
The goal of this contingency plan is to provide detailed information to Public Works employees in the event
of a failure at a wastewater pumping station such that the employee(s)can take immediate action to prevent
a potential SSO from occurring. This document will protect the citizens and environment of Elizabeth City,
NC by establishing written procedures for responding to a pump station failure.
CONTINGENCY PLAN DEVELOPMENT
It is recognized that the various duplex and simplex pumping stations may have different pumps,valves, and
other components that will influence the contingency procedure.Not only components but configuration and
condition of the station must be taken into account. Therefore,a specific contingency plan may be
applicable to one or more pumping stations. The following attachments are contingency plans for pump
station failure. Attachments also include emergency personnel and contractors'phone numbers. The
applicable contingency plan and phone list shall be posted at each pumping station and used to correct loss
of wastewater flow and prevent an SSO. The following is the process by which the contingency plan(s)was
developed.
1) Establish the attributes of each pumping station,e.g. type of station(duplex or simplex),pump/motor
status/condition,whether a by-pass connection is available, if a stand-by generator or pump is
installed, etc.
2)Using the attributes of each pumping station, determine the hazards that could cause a pump station
failure.
3) Based on the pumping station attributes and hazard analysis, determine the most effective means to
restore pump capability and/or mitigate the possibility of sanitary sewer
Overflow.
4)Develop the contingency procedure for the pumping station and post at the station.
Main Station-This station was completely remodeled in 2005. Station has two new pumps and two rebuilt
pumps all new check valves and gate valves. Station also has bypass capability and has a built in generator
that automatically comes on when power is lost. Hazards: Failure of operational pump or motor;power
failure/generator failure; failure of control panel components; bubbler system malfunction;piping/pump
blockage.
Pearl Street-Older pump station has built-in connection for generator. Station has back up emergency pump
installed that runs on natural gas. The emergency pump will automatically come on if the power to the
station is lost or if the level inside of wet well exceeds its normal level due to primary pump failure.

Williams Circle-This station has two old suction lift pumps that are very difficult to find parts for;the
check valves are questionable; station has by-pass capability and has built in connection for generator.
