Dear Ms. Eplin:
Dewberry Engineers Inc. is requesting a modification to a previously approved (Permit No. GUILF-2024-
0115) Erosion & Sediment Control Plan set. The modification is a result of changes in Duke Energy
Carolinas (DEC) design. Another sheet was added to include new work along Kendale Rd (SR 1834), and
other minor design changes were also made along the original alignment. Total limits of disturbance
increased from 13.6 acres to 14.3 acres. Included in this submittal are:
• $100 Application Fee (Check No. XXXX)
• One (1)copy of the Erosion Control Narrative
• One (1) original and one (1) copy of the completed and notarized Financial
Responsibility/Ownership Form
• Two (2)sets of Drawings
