Please provide the following information by 09/14/2024, and note that the requested information must be submitted within 30 days of this request, or the applicant will be required to submit a new application and application fee in accordance with 15A NCAC 02T.0107. 1.This application is missing a narrative. 2. Section VI.6 checked yes, but no flow reduction approval letter was attached. 3.Table VI.7 should mention establishment type in details regarding number of bedrooms as per 02T.0114 (f) unless a flow reduction approval letter is submitted. You added WQ0030925 flow within this permit. If you are planning to abandoned that permit, all facilities should be described in this table and in narrative under WQ0030925.Your narrative and map should also explain the length of sewer replacement, use of existing sewer, and rescinding portion of the existing permit. This is critical and need further explanation. I would be happy to setup a zoom meeting if you want. 4. Section V of the FTA should match USE section II. Downstream Facility Sewer Name and permit number. WQ0031004 MOD is listed as the pump station on FTSE.Your map says WQ0030925 will be removed. Please explain. 5. Section IX.S. indicates that a 401/404 authorization is needed. Please provide the Permit Number/Permitting Status letter for the 404/401 authorization specified in Section IX.S. of the Fast Track Application. This information can be entered into a comprehensive narrative. 6. No topographical map submitted and the aerial map provided is not sufficient. It does not show project area, GS, direction of flow, downstream connection properly.According to Section F of the Application Instructions, an 8.5-inch x 11-inch color copy of a USGS Topographic Map and an aerial map should be included with the application. The maps need to clearly identify the project area, including the closest surface waters, general location of the gravity sewer, pumpstations, and force mains, and the downstream connection points for the receiving sewer. Please provide the required maps according to the application requirements. 7. You submitted same map for two of the applications (North and South) which says WQ0030925 to be removed. Is this the correct map? 8. WQ0030925 is listed as the downstream receiving sewer, however, your map says "private pump station to be removed". Please explain. 15A NCAC 02T.0107 Staff Review and Permit Preparation states: • "(2) (e) If an application is accepted and later found to be incomplete, the applicant shall be advised how the application or accompanying supporting information may be modified to make it complete. The staff shall advise the applicant: (2) if all required information is not submitted within 30 days, the project will be returned as incomplete. Any resubmittal of a returned application shall be accompanied with a new application fee" Please contact me with any questions that you may have regarding this request. *Please note that the fee for fast track sewer extension permits has increased to$600 as of October 3,2023[as stipulated in the 2023 House Appropriations Act, House Bill 259, Water Quality and Stormwater Fees,Section 12.14(a)].
