A review was conducted, and the permit application is deemed incomplete. Per Session Law 2024-49 Senate Bill 166 (Section 4.3), information needed for a complete permit application must be submitted within 30 days of this email or the application will be returned to the applicant, however the fee will not be returned. Please review items that are missing or need correction from the permit application below. I.3: Jim Koffman is not listed as an officer or managing member on the Secretary of State’s website for First Victory Inc. Please provide proof of delegation of signature authority to Mr. Koffman from a listed principal officer of the company or please resubmit the application signed by a listed officer of the company. IV.1: Please revise this section to list the correct receiving facility and owner of the sewerage system. Facility- MSD Water Reclamation Facility and Owner: MSD VII: The narrative indicates the private collection system will connect to a new public system installed by the same developer and indicates the FTA is for 1,272 LF of 6” line; however, section VII of the application also includes 978 LF of 8” line. Please verify if this was included in error and if the public 8” downstream line will be permitted under a separate FTA. FTSE: Please verify the flow under section I.f of the FTSE is meant to account for the entire Northridge Farms project and not just the 100 units being permitted under this FTA. If this is not the intent on the FTSE, please revise. Attached to this email is an electronic copy of the application form and supporting documents submitted. Once you have obtained the necessary information, please submit an electronic copy of the requested item to complete the administrative review. You will be notified via email once the information is received. When sending correspondences regarding your FTSE permit application please include the project name in the subject line.
