JAY ZIMMERMAN ENVIRONMENTAL QUALITY February 5, 2016 Director Mr. Jay Meyers Public Utilities Director Town of Fuquay-Varina 401 Old Honeycutt Road Fuquay-Varina, North Carolina 27526 SUBJECT: Authorization to Construct A to C No. 066516A03 Town of Fuquay-Varina Terrible Creek WWTP Dear Mr. Meyers: A letter of request for an Authorization to Construct was received December 16, 2015, by the Division of Water Resources (Division), and final plans and specifications for the subject project have been reviewed and found to be satisfactory. Authorization is hereby granted for the construction of modifications to the existing 1.0 MGD Terrible Creek WWTP, with discharge of treated wastewater into the Terrible Creek in the Neuse River Basin. This authorization results in an increase in design and permitted capacity to 3.0 MGD, and is awarded for the construction of the following specific modifications: Demolition of existing influent wet wells, existing clarifiers # 1, #2, #3, existing tertiary filters #1, #2, #3, existing blowers, existing chemical storage facility, existing post aeration basin, existing UV disinfection system, and other surplus components; refurbishment of existing treatment plant #1 and #2 basins to two (2) 0.383 MG EQ basins, piping, and controls; refurbishment of treatment plant #3 basin to one (1) 0.682 MG EQ basin, piping, and controls;installation of new influent pump station with 18 MGD peak hourly mechanical and manual coarse screen system and incline conveyor, consisting of four (4) submersible 1,565 GPM at 58 ft TDH pumps with VFD, two (2) submersible 781 GPM at 58 ft TDH pumps with VFD, influent composite sampler, piping, and controls; Installation of new preliminary treatment facility consisting of two (2) 4.5 MGD peak hourly mechanical step fine screens systems, a manual screen channel,two (2) 4.5 MGFD peak hourly grit removal systems with cyclone/classifier and screening compactor,a manual grit bypass channel,three (3) 300 GPM at 55 ft TDH grit pumps, piping, and controls; installation of two (2) new 85 ft diameter clarifiers with 200 GPM at 28 TDH scum pumps, piping, and controls; installation of new RAS/WAS pump station consisting of three (3) 1,389 GPM at 28 ft TDH pumps with VFD, piping, and controls; installation of four (4) new blowers with VFD, 2 blowers each 1,410 SCFM at 15 psig maximum and 2 blowers each 1,100 SCFM at 15 psig maximum, piping, and controls; installation of two (2) new 309 ft2 tertiary disk filters systems with each filter system peak flow rated at 4.5 MGD and consisting of internal backwash pump system and wash system, multiple disks, piping, and controls; installation of new dual channel UV system with UV intensity and lamp monitoring and automatic lamp sleeve cleaning, system rated for disinfection of 9.0 MGD to meet NPDES fecal coliform limits per North Carolina minimum UV design criterion, 3 banks in-series per channel, 8 lamps per bank, with one bank per channel designated as stand by, and other controls; installation of one (1) new 0.4 MG Sludge Storage tank with fine air bubble diffuser array, piping, and controls; installation of new sludge dewatering and truck loadout system consisting of two (2) 400 GPM at 70 TDH Dewatering Sludge Feed pumps, two (2) adjustable polymer addition and mixer systems design for maximum 1,500 GPH of 2% polymer solution, two (2) integrated independent gravity belt thickener with sludge hopper and screw conveyor and continuous belt filter press , two (2) 100 GPM at 115 ft TDH TWAS pumps,three (3) in series dewatered sludge screw conveyors designed for maximum 24,000 lbs/hr, contained truck loading area, piping, and controls; installation of new chemical storage area and chemical feed systems with containment for each chemical tank system and for the corresponding truck unloading area, consisting of one (1) 5,000 gallon ALS tank with three (3) 111 GPH at 30 psig adjustable chemical feed pumps and containment sump with pump, one (1) 13,000 gallon caustic -storage tank with water dilution meter and two (2) 110 GPM at 20 TDH circulation/mixing pumps and two (2) 111 GPH at 30 psig adjustable chemical feed pumps and containment sump with pump,one(1)7,300 gallon SCS tank with three(3) 111 GPH at 30 psig adjustable chemical feed pumps and containment sump with pump,piping, and controls; installation of one (1) diesel driven 1,000 KW, 480 vac, 3 phase standby generator system with 24 hr capacity fuel tank, and controls; in conformity with the project plans, specifications, and other supporting data subsequently filed and approved by the Department of Environmental Quality. This Authorization to Construct is issued in accordance with Part III, Paragraph A of NPDES Permit No. NCO066516 issued effective January 1, 2016, and shall be subject to revocation unless the wastewater treatment facilities are constructed in accordance with the conditions and limitations specified in Permit No. NC0066516. The sludge generated from these treatment facilities must be disposed of in accordance with G.S. 143-215.1 and in a manner approved by the Division. Mr. Jay Meyers, Public Utilities Director Town of Fuquay-Varina February 5, 2016 Page 3 of 4 In the event that the facilities fail to perform satisfactorily, including the creation of nuisance conditions, the Permittee shall take immediate corrective action, including those as may be required by the Division, such as the construction of additional or replacement wastewater treatment or disposal facilities. The Raleigh Regional Office,telephone number(919) 791 -4200, shall be notified at least forty-eight(48)hours in advance of operation of the installed facilities so that an on site inspection can be made. Such notification to the regional supervisor shall be made during the normal office hours from 8:00 a.m. until 5:00 p.m. on Monday through Friday, excluding State Holidays. Upon completion of construction and prior to operation of this permitted facility, a certification must be received from`a professional engineer certifying that the permitted facility has been installed in accordance,with the NPDES Permit, this Authorization to Construct and the approved plans and specifications. Mail the Certification to: Division of Water Resources, WQ Permitting Section—NPDES, 1617 Mail Service Center, Raleigh,NC 27699-1617. Upon classification of the facility by the Certification Commission, the Permittee shall employ a certified wastewater treatment plant operator to be in responsible charge (ORC) of the wastewater treatment facilities. The operator must hold a certificate of the type and grade at least equivalent to or greater than the classification assigned to the wastewater treatment facilities by the Certification Commission. The Permittee must also employ a certified back-up operator of the appropriate type and grade to comply with the conditions of T15A:8G.0202. The ORC of the facility must visit each Class I facility at least weekly and each Class II, III.and IV facility at least daily, excluding weekends and holidays, must properly manage the facility, must document daily operation and maintenance of the facility, and must comply with all other conditions of T15A:8G.0202. A copy of the approved plans and specifications shall be maintained on file by the Permittee for the life of the facility. During the construction of the proposed additions/modifications, the permittee shall continue to properly maintain and operate the existing wastewater treatment facilities at,all times, and in such a manner, as necessary to comply with the effluent limits specified in the NPDES Permit. You are reminded that it is mandatory for the project to be constructed in accordance with the North Carolina Sedimentation Pollution Control Act, and when applicable,the North Carolina Dam Safety Act. In addition, the specifications must clearly state what the contractor's responsibilities shall be in complying with these Acts. Prior to entering into any contract(s) for construction, the recipient must have obtained all applicable permits from the State. The issuance of this Authorization to Construct does not preclude the Permittee from complying with any and all statutes, rules, regulations, or ordinances which may be imposed by other government agencies (local, state, and federal) which have jurisdiction.
