The change of ownership form should have been included in the submittal, see attached for convenience.
I believe in the past we have done that and the certification in the same step but I could be wrong as it has
been a few years since we had both on the same project.
If we need to submit this separately to Matthew is the file attached adequate or do we need to submit the
original document?

Good morning,
I hope all is well. I am currently reviewing the certification referenced in the subject line above.
Before I can continue with the certification review process, the permit must be modified in
order to reflect the ownership change. This is done in the central office located in Raleigh; I
have cc'd Mathew Nevills who oversees ownership changes. I've also attached a copy of the
certification form in this email which includes the transfer of ownership application. Once the
transfer of ownership is officially done, please email me a copy of the new permit.
