Complaint regarding pipe at end of swale#12 and rip rap apron on end of swale#10. The low density subdivision cannot
use pipe to convey water except for driveways. All runoff conveyances must be vegetated with a 3:1 (h:v)slope.

is in violation of permit design standard 4 and 5. Swale#10 has a rip rap spreader that extends to the
wetland ditch, also violating design standard 5.

This permit has been under violation (NOV-2009-PC-0097) failure to maintain the stormwater system. As of this
inspection, the swales have been repaired and re-vegetated in accordance with the O&M agreement and are deemed
complaint.

Comment: Swales have been repaired, soded and are in compliance with the permit.
It was noted during the inspection that sod should be secured with landscaping stakes
for the first 10-15 feet and seed should be added to the north side of Swale no. 12.

Inspection Summary:
This project continues to be under a Notice of Violation (NOV-2009-PC-0097). The following deficiencies were observed:
1. Some swales within the project have ponding and cattails/phragmites established; swales appear to have improper
slopes
2. No maintenance of swales was evident with observance of overgrown swales, bottoms of many of the swales are not
properly vegetated.
3.A written certification from the Permittee stating the project's stormwater controls, and impervious surfaces have been
constructed within substantial intent of the approved plans and specifications was not found in the file.
In order to bring this state stormwater permit into compliance, please complete the following:
1. Repair swales to the approved plans and specifications of the permit. •
2. Perform maintenance of all swales to meet the O&M agreement requirements.
3. Submit a written certification of the stormwater system by the Permittee to this office, (II. Schedule of Compliance,
No.7, page 3).

Inspection Summary:
In order to bring this state stormwater permit into compliance, please complete the following:
1. Maintain swales to include removal of accumulated sediment,regrade to meet slope requirements from the approved
permit,and reseed to establish centipede sod as noted on the approved permit. Mow all swales to keep vegetation under
6 inches in height. Repair all areas of erosion within the swales.
2. Remove temporary check dams from swales.
3.Submit a copy of the recorded deed restrictions to this office. Per the requirements of the permit, a copy of the
recorded deed restrictions must be forwarded within 30 days of the date of recording.

Comment: The approved plan shows centipede sod is to be installed in vegetated
swales. The swales are overgrown, and the bottoms of many of the swales are not
properly vegetated. Temporary ditch checks remain at the end of swales.
