The subject permit became effective March 1, 2004 and will expire March 31, 2008. Please
ensure to apply for a permit renewal by October 31, 2007 to avoid civil penalty assessment.

The Water Pollution Control System Operators Certification Commission has classified the
subject facility as a Grade 1 Physical / Chemical System. As required by Rule 15A NCAC 8G
.0202(2) and the subject permit, a certified grade one Physical / Chemical (PC-1) Operator in
Responsible Charge (ORC) and a back-up ORC must be designated for this system. Upon
inspection of the facility and conversing with you, it was determined that the 2-oil/water
separators do not discharge any product. This facility does not require a PC-1 classification and
therefore a PC-1 ORC is not required. A request to rescind the permit must be made in writing.
Please mail your request to Technical Assistance and Certification Unit, 1618 Mail Service

The facility consists of 2 oil / water separators, product holding tank, and a 12,000 gallon
wastewater holding tank. Any product that leaves the covered loading rack area flows thru the
two oil water separators. Any product recovered from the oil / water separators flows to the
product holding tank and is hauled off as needed. Any water captured from the oil / water
separators flows to the wastewater holding tank in the bermed area containing 5 above ground
storage tanks. The wastewater in the holding tank is pumped out the same time the product
holding tank is pumped out. Stormwater that accumulates in the diked area is discharged into a
small UT to Mill Creek.
The subject inspection was conducted by myself on January 24, 2007. Your assistance and
cooperation with this inspection was appreciated. Please find the attached Basin Wide Information
Management System (BIMS) inspection checklists summarizing the inspection. Some items noted
during the inspection are listed below:

There was very little rainwater within the diked area. The water appeared clear with no
discernible sheen.

4. There has been no discharge from this facility during December 2005 to December 2006. All
Discharge Monitoring Reports (DMR’s) for that period indicated No Discharge. There was no
analytical data to check. One Notice of Violation was issued for reporting a late DMR for July
2006.

The treatment facility consists of 2 oil/water separators, product holding tank, and 12,000-gallon wastewater
tank. Wastewater generated from the loading rack area flows through the oil/water separators with the water
part flowing to the wastewater tank, located in the bermed area, and the oil part flowing to the product
holding tank. Shamrock Environmental pumped out the wastewater-holding tank, the product-holding tank
on October 28, 2006. There is no discharge from the 2-oil/water separators. This facility does not require an
PC-1 classification. A request to rescend the PC-1 classification should be made to the Technical
Assistance and Certification Unit. There are secondary containment walls surrounding the additive tanks. A
diked wall surrounds the AST's on site. There has not been a discharge from the diked area for the 2005 -
2006 year.
One Notice of Violation was issued for July 2006 for a late DMR. There were no other permit violations.
