Dear Mr. Harrell:
Steve Mauney of the Winston-Salem Regional Office of the NC Division of Water Quality
(DWQ or the Division) conducted a compliance evaluation inspection (CEI) at the subject facility on
May 16, 2012. We were able to locate the chlorinator and the discharge point. An inspection
checklist is attached for your records and the inspection findings are summarized below.
Mr. Mauney detected no problems with the system. Please continue to have the septic tank
pumped every three to five years when the cabin is being used regularly as required by the permit
and please also check your chlorinator at least once per month to ensure it has enough chlorine and
that it hasn't clogged up. You should also ensure your chlorine tablets are rated for wastewater
treatment, not for use with swimming pools (this is normally indicated on the container). Wastewater
rated tablets can be acquired from one of the supply sources on the attached list or other local
suppliers. No flow was detected from the effluent pipe during the inspection. Annual sampling of the
effluent discharge should be conducted as required by the permit if a discharge occurs. You should
also document your operation/maintenance of the system and keep all records for a period of three
years as you have been doing and maintain records for the annual samples. Your efforts to maintain
the system properly are appreciated.
The Certificate of Coverage (NCG550288) was renewed in 2007 and will expire July 31, 2012.
Please be aware that the Division sends out invoices for the annual fee. Of course, the invoices must
be paid (annual fee currently $60/yr). It is understood that a new Certificate of Coverage will be sent
out in a few months.

Comment: No tests have been conducted as no discharge has been observed in
recent years. The use of the cabin is minimal except for ocassional cookouts (ie. cabin
used 3 nights in recent years). The cabin is down near the stream in the side yard of
the Harrell residence. The cabin has one bedroom and bathroom, a kitchen sink and a
pump tank and force main back to the septic tank-sandfilter system.

Comment: The septic tank and subsurface sand filter are not accessible to the general
public. A staff report from 1988 indicates the sand filter is 261sq. feet to comply with the
minimum 2 bedroom design requirements.

Comment: Mr. Harrell indicated the tank was pumped 3 years ago (2009) during
additional construction activity at their primary residence. The Harrell residence has 2 lift
pumps with a subsurface leach field up along the upper edge of their property. The
Harrell residence was constructed near the location of the former cabin owned by Tyre
Dodson. The current cabin is appx 200 feet from the Harrell residence and adjacent to
the creek.

Comment: The cabin is not occupied and the sewer system is only used infrequently
by anyone. There did not appear to be any chlorine in the tubes but this deficiency is
mitigated by the fact there was no discharge from the system.

Comment: The effluent pipe extends appx. 90-120 feet from the chlorinator to the
bank of the receiving stream. The pipe was dry with soil partially blocking the outlet from
the pipe.
