Dear Mr. McDaniel:
A letter of request for Authorization to Construct was received June 8,
1984, by the Division and final plans and specifications for the subject
project have been reviewed and found to be satisfactory. Authorization is
hereby granted for the construction of a 450 GPD wastewater treatment facility
consisting of a 1000-gallon baffled septic tank, distribution box, 396 S.F.
subsurface sand filter, chlorine contact tank with tablet-type chlorinator,
and associated piping and appurtenances to serve the Walter R. McDaniel
Residence with discharge of treated wastewater into a common effluent line
with rip-rap or a cascade aerator at the final discharge point.
This Authorization to Construct is issued in accordance with Part III
paragraph C of NPDES Permit No. NC0059463, issued June 1, 1984, and shall be
subject to revocation unless the wastewater treatment facilities are con-
structed in accordance with the conditions and limitations specified in
Permit No. NC0059463.
The sludge generated from these treatemnt facilities must be disposed of
in accordance with G.S. 143-215.1 and in a manner approvable by the North
Carolina Division of Environmental Management.
The Winston-Salem Regional Office, telephone number 919/761-2351, shall
be notified at least twenty-four (24) hours in advance of backfilling of the
installed subsurface filter system so that an in-place inspection can be made
of said system prior to backfilling. Such notification to the Regional
Supervisor shall be made during the normal office hours from 8:00 A.M. until
- 5:00 P.M. on Monday through Friday, excluding State Holidays.

In event the facilities fail to perform satisfactorily in meeting its
NPDES permit effluent limits, Mr. Walter R. McDaniel, Owner shall take such
immediate corrective action as may be required by this Division, including
the construction of additional wastewater treatment and disposal facilities.
The sand media of the subsurface filter must comply with the Division's
sand specifications and must be analyzed and approved by this Division either
by direct sampling or by acquisition of filter sand from a dealer who is
currently certified by the Division as an acceptable source.
One (1) set of approved plans and specifications is being forwarded to
you. If you have any questions or need additional information, please contact
