SFR Single-Family Residence Wastewater Irrigation System Person County Dear Mr. Garcia: The Division of Water Resources (Division) has noted that the subject single-family residence wastewater irrigation facility has changed ownership. The Alamance County Register of Deeds indicates Edgar Geovany Garcia purchased the subject property (Parcel No. 9818575158) from Dimas A. G. Medrano and Stacy J. Medrano on October 24, 2022 (Deed Book: 4409 / Page: 175). The wastewater treatment and irrigation system associated with this property is covered under an individual State-issued non-discharge permit (Permit No. WQ0010420). Please complete and submit the attached Change of Ownership application (Form: SFRWWIS-CO 03-21). This form has been prefilled for your convenience. Please complete any blank portions and double-check all information for accuracy. Within 30 calendar days of receiving this notification, submit the completed application to: Email: Mail: Non-Discharge.Reports@deq.nc.gov Attn: Alys Hannum OR Non-Discharge Branch 1617 Mail Service Center Raleigh, NC 27699-1617 Once the Division receives a Change of Ownership application, Raleigh Regional Office staff may conduct a site visit and provide an overview of the treatment and disposal system. Upon completion of the site visit and review of the application, the Division will issue a new non-discharge permit for a length of up to eight years. Garcia Notice of Deficiency Page 2 of 2 Failure to provide this form may result in a Notice of Violation for operating the subject facilities without a valid permit, which is a violation of G.S. 143-215.1 and may subject you to appropriate enforcement actions pursuant to G.S. 143-215.6A, 6B, and 6C. The Division may assess civil penalties of up to $25,000 per day per violation for failure to secure a valid permit.
