The Division of Energy, Mineral, and Land Resources (DEMLR) received a complete Stormwater Management
Permit Application for the subject project on October 30th, 2024. Staff review of the plans and specifications
has determined that the project, as proposed,will comply with the Stormwater Regulations set forth in Title 15A
NCAC 21-1.1000 - 1062 rules.We are forwarding Permit No.SW3240203, dated October 30th, 2024,for the
construction,operation and maintenance of the built-upon areas (BUA)and vegetated conveyances or areas
associated with the subject project.This permit shall be effective from the date of issuance and shall be subject
to the conditions and limitations as specified therein and does not supersede any other agency permit that may
be required.
The original signed permit package and approved plans are being mailed to the permittee, Union Shrine
Properties, LLC, at the following address (NOTE:An electronic version of this permit can be found in our online

Jacob,
I responded to your request for written permission to make the
revisions on September 5, 2024, asking you to please make any
corrections as you offered. That response is included in this
email chain. In reviewing the submittals, I don' t see where the
V actual (velocities) are not in agreement with the calculations
we submitted. As you said, and as we discussed by telephone,
this is a minor change which with my permission, you can
revise. Following my September 5, 2024 email requesting that
you to make the minor corrections, I thought this was sorted
out. I have attached the latest forms we submitted herein for
your convenience. I have reviewed the forms and cannot locate
the discrepancy.
However, and to be clear: Please make the adjustments to the
Supplemental EZ form V actual velocity column to match our
calculations. Please continue with the permit process / issuance
and let me know if anything further is required.

I looked over the additional information you provided on June 4, 2024. Everything looks good except for a small
issue:
Please correct the following:
• The Vactual column in the newly submitted Supplement-EZ does not reflect what is shown in
the updated calculations. Please revise for consistency.
As this is the only issue,and a minor one,with your written permission I can edit our digital/physical
copies of the Supplement-EZ to reflect this change. If this is desired, please respond to this email
indicating so and I will make that change.
Please provide one hard copy and one electronic copy of any items needed to address these comments, unless
specified otherwise.

the Standard permitting process can take up to 90 calendar days to complete. You can see where this project stands in

on it). Please upload all documents that you mailed into our office through the link provided below. Be sure to choose
the Central Office as the Primary Contact office.

IMPORTANT NOTE: Please ensure that each file is uploaded under its appropriate file type. Additional file types can be

CONSTRUCTION FILE UPLOAD"TO ADD ADDITIONAL FILE TYPES TO THE SUBMISSION. Merely changing the file type

is not administratively complete and cannot be accepted for review in its current condition. Please provide the
following submittal items by close of business on 2/29/24, so that we may accept the project and begin the review
process, otherwise, the project will be returned as administratively incomplete.
• 2x hard copies of the Application with the following corrections made (These items are required per 15A NCAC
02H .1042(2)(a) and per Section VI, 1 of the Application): Please see the Application Instructions attached.
o Section IV, 8—The percent impervious area for the project area does not appear to be calculated
correctly. Project density calculation information can be found in 15A NCAC 02H .1003(1).
o Section IV, 10—The value calculated for"% Impervious Area" is incorrect. Please revise.
o Section IV, 10—The value given for Proposed Impervious Area (sf) and Total (sf) should match. Please
revise.

o Section IV, 6—There do not appear to be any surface water on-site according to the USGS map. Please
revise this value and the value for Section III, 7.
o Section IV, 8—The percent impervious area for the project area does not appear to be calculated
correctly. Project density calculation information can be found in 15A NCAC 02H .1003(1).
o Section IV, 10—Please use the information for the first named surface water("13-17-40-5")for the
receiving stream index number.

o Section IV, 10—Double check the drainage area.This often is not the same as the total project area and
should be based off of topography.
■ Additionally, if there is offsite area draining onsite, the offsite area should be listed as 100%
impervious unless you can divert it around the site.
o Section IV, 10—The value calculated for"% Impervious Area" is incorrect. Please revise.
• 1x hard copy of sealed, signed, and date calculations relevant to the application's project.This item is required
per 15A NCAC 02H .1040(2)(f) and per Section VI, 7 of the application. Provided calcs were not signed or
sealed. For calculations, we need impervious area and BUA calcs, calcs for the outlet to show that it is non-
erosive in a 10-year storm for the time of concentration specific to the outlet area, and if the swales are going
to stay and have a permanent liner(grass) in place, then calcs to show that these swales are non-erosive.
• 1x hard copy of the complete, original signed Supplement-EZ Form (This item is required per Sections V&VI, 3
of the Application).This form can be found on our website at the following address:

o Existing site conditions.This item is required per Section VI, 8k&8n of the Application and 15A NCAC
02H .1042(2)(g)(v).
o Wetland delineation or a note on the plans stating that there are no wetlands located on-site.This item
is required per 15A NCAC 02H .1042(2)(g)(iii) &Section VI, 8m of the Application. Please also list the
name of the individual that made this determination in a note on the plans.
o BUA dimensions and layouts.This item is required per Section VI, 8j of the Application.The BUA
information shown in the Application, Supplement-EZ Form, and calculations should be verified from
the plans. Please provide more dimensions so that we can certify the BUA on the site plans.
o Details of roads, sidewalks, drainage features, and piped collection systems.This item is required per
Section VI 81 of the application. Show cross section of driveways, (if applicable) curbs and gutters,
berms, ditches/swales if permanent.
o Clearly show what are temporary vs. what are permanent conditions (only what will be maintained in
perpetuity).
■ Show proposed grading so that we can see how sediment basins and ditches/swales are to be
filled.
NOTE:This is an administrative completeness review, not a detailed engineering review, there may be other issues
that may be brough up during the engineering review.
