Facility and Address: HNiGM- SUppNT BRANCH Mr. Paul H. Latham (Lot #14 Tax Block 420 (formerly Mr. Tommy Anthony, SFR) 6911 Mendelssohn Drive , Clemmons, NC 27012 2. Persons Contacted and Telephone Number: Mr. Bob Westerberg, Builder, ph 724-3254 ( 1606) and 924-6665 (Home) 5. Directions to Site: Take Middlebrook Drive off of Hwy 158 in Clemmons. Proceed 0. 3 mile and then turn right onto Greenbrook Drive, left onto Breckinridge Lane and then right onto Mendelssohn. The Latham SFR is the first on the right. 6. Discharge Point - Latitude: /? 80° 23 ' 14" Longitude: �? 36° 00 ' 53" Attach a USGS Map Extract and indicated treatment plant site and discharge point on map. USGS Quad No. C17SW or USGS Quad Name Clemmons, NC 7. Size ( land available for expansion and upgrading) : There is adequate land available for expansion. 8. Topography (relationship to flood plain included) : The system is to be located in a low area adjacent to the creek. The flat terrain makes installation of dual filters unfeasible 9. Location of nearest dwelling: The residence is in a subdivision with other dwellings located nearby 10. Receiving stream or affected surface waters: U.T. to Johnson Creek • a. Classification: WS-III b. River Basin and Subbasin No. : YAD-02 c. Describe receiving stream features and pertinent downstream uses: The U.T. to Johnson Creek borders the Clemmons West subdivision. Numerous single family residence systems discharge into this tributary. PART II - DESCRIPTION OF DISCHARGE AND TREATMENT WORKS 1. Type of wastewater: 100% Domestic % Industrial a. Volume of Wastewater: 450 GPD permitted flow. (System can be designed for 360 GPD based on 120 gpd/bedroom) b. Types and quantities of industrial wastewater: N/A c. Prevalent toxic constituents in wastewater: N/A d. Pretreatment Program (POTWs only) N/A in development approved should be required not needed 2. Production rates ( industrial discharges only) in pounds N/A a. highest month in the last 12 months b. highest year in last 5 years 3 . Description of industrial process ( for industries only) and applicable CFR Part and Subpart: N/A 4. Type of treatment ( specify whether proposed or existing) : Plans of the proposed system2show the following: 1000 gallon septic tank, a 450 ft subsurface sandfilter, chlorinator and contact tank, step aeration device and discharge pipe to the creek. 5. Sludge handling and disposal scheme: Sludge should be removed regularly from the septic tank by a licensed septic tank pumper to be disposed of in the nearest municipal WWTP. j 6. Other: PART IV - EVALUATION AND RECOMMENDATIONS The permit was originally issued to Mr. Tommy Anthony on August 21, 1984, however, no residence was ever constructed. Forsyth Builders (Mr. Bob Westerberg) purchased the lot in June 1989, and began construction of a three bedroom residence shortly thereafter. The home is basically complete and will be ready for occupancy on October 1, 1989, by Mr. Paul Latham. Engineering plans and specifications were submitted to Raleigh on September 11, 1989. The plans appear adequate. It should be noted that installation of a secondary subsurface sandfilter is unfeasible due to inadequate slope. For this reason the size of the filter has been increased to 450 ft . WSRO recommends transfer of ownership and reissuance of NPDES permit NO. NC0059170 to Mr. Latham. This area of Clemmons is scheduled to receive municipal sewer prior to December 1990. Since the System discharges into a "zero" flow stream, the permit should expire on December 31, 1991. In addition, the permit should contain the standard clause requiring connection to a POTW within 6 months of its availability.
