We have neither the staff nor the$$$to mail out renewal applications this year.
Just send him the attached form. It's easier to complete than the name&ownership change form.

CHARLES-This system and property is now owned by Scott Rakestraw who bought it from his mother.The address will
not change.Will you guys be sending out renewal forms or will the new COC's just be reissued to the current permittee?
I can send him the change of ownership form with my recent inspection form.Thanks for the help.
