Dee Freeman Governor Director Secretary May 7, 2012 RECEIVER MAY 16 2012 Mr. Scott Rakestraw 751 Eagle Falls Road CENTRAL FILES Madison, North Carolina 27025 DWQ/BOG SUBJECT: Notice of Deficiency & Compliance Evaluation Inspection Single Family Residence Wastewater Treatment System NPDES General Wastewater Permit NCG550000 EC e rt if i cate-of-Coverage=#-N C G 5 5 02 6-0 Rockingham County Dear Mr. Rakestraw: Steve Mauney of the Winston-Salem Regional Office of the NC Division of Water Quality (DWQ or the Division) conducted a compliance evaluation inspection (CEI) at the subject facility on May 3, 2012. We were able to locate the septic tank, the chlorinator and the discharge point. Chlorine tablets were present in the chlorinator tubes. An inspection checklist is attached for your records and the inspection findings are summarized below. Mr. Mauney detected no problems with the system. A very small flow was detected from the effluent pipe. Please remember to have the septic tank pumped every three to five years as indicated • in the permit and please also check your chlorinator at least once per month to ensure it has enough chlorine and that it hasn't clogged up. You should also ensure your chlorine tablets are rated for wastewater treatment, not for use with swimming pools (this is normally indicated on the container). Wastewater rated tablets can be acquired from one of the supply sources on the attached list or a local supplier. You should also document your operation/maintenance of the system and keep all records for a period of three years. Finally, you are reminded of the requirement to have the discharge tested by a NC certified laboratory once per year, as long as the system has a discharge. Please notify this office when sample results are received. The Certificate of Coverage (NCG550268) was renewed in 2007 to Sarah Rakestraw, your mother. You must now apply for this Certificate of Coverage (form attached) as you have now purchased the property. This should be accomplished by May 21, 2012. Please complete the attached form and attach a copy of the last septic tank service receipt. Discharging wastewater without a Permit or Certificate of Coverage can result in civil penalties. Please be aware that the Division sends out invoices for the annual fee (currently $60/yr).
