Dear Mr. & Mrs. Cafferata:
Jenny Graznak of the Winston-Salem Regional Office performed a Compliance Evaluation
Inspection on the wastewater treatment system at your property in Alamance County, North Carolina
on November 18, 2008. This type of inspection consists of two basic parts: a review of self-
monitoring data, and an on-site inspection of the facility and its discharge. You were not home
during either of Ms. Graznak's visits to your property. Attempts to contact you to schedule this
inspection were unsuccessful. No one was home during the inspection.
You hold a General Permit to discharge Domestic Wastewaters from Single Family
Residences under the National Pollutant Discharge Elimination System (NPDES). The discharge
from this system discharges to an unnamed tributary to Big Alamance Creek. Permit text Part I •
Section A (attached to this correspondence) specifies the limits and monitoring requirements for your
permit. The following parameters must be monitored on an annual basis: Flow, Biochemical Oxygen
Demand (BOD), Total Suspended Residue, Fecal Coliform, and Total Residual Chlorine. A
Laboratory with North Carolina Wastewater Certification (a list of approved laboratories is attached)
must perform this monitoring. You must also conduct and document the. following maintenance
activities:
• Septic tanks shall be maintained at all times to prevent seepage of sewage
• Septic tanks will be checked at least yearly to determine if solids must be removed
• Septic tanks shall be pumped out within 3-5 years of issuance date of the Permit.
Contents removed from septic tanks shall be disposed of at a location and in a manner
compliant with all local and state regulations
• Disinfection apparatus shall be inspected weekly
The chlorinator tubes are located in the front left corner of your property. Evidence of chlorine
tablets was not observed during the inspection. The discharge pipe runs across the neighbors' yard
to a culvert under Nealwood Avenue. The white PVC discharge pipe is located approximately 10
feet upstream of the culvert pipe. No discharge was observed during the inspection. If you have
