Enclosed please find a copy of the Compliance Evaluation Inspection Report for the
inspection we conducted at the West Jefferson WWTP on September 10, 2024. Please note that
in the stormwater inspection report Summary we ask that you create and maintain a Stormwater
Pollution Prevention Plan, conduct quarterly analytical monitoring, and print a copy the current
permit and Certificate of Coverage to keep on site.

Inspection Summary:
On September 10th, 2024, this facility was inspected for compliance.We met with Brandon Patrick on site. During the
inspection the following issues were noted:
1. SWPPP has not been created or maintained on site. A SWPPP shall be created and maintained in accordance with
NCG 110000.
2. No qualitative monitoring records on site. Qualitative mornitoring should be conducted quarterly in accordance with
NCG 110000.
3. No copies of current permit and COC on site.A copy of the current permit and COC should be available at the site at all
times.
Analyze and address appropriately.
Please give the above items attention and the facility will be reinspected on or after November 23, 2024.
