Box 1388 Lumberton, NC 28359 Dear Mayor Weinstein: August 17, 1989 Weinstein, Mayor SUBJECT: Compliance Inspection Permit No. 15218 Land Application of Sludge Facility Town of Lumberton Robeson County On August 9, 1989, an inspection of the Town's land application of sludge facility was performed by Mr. Grady Dobson, Environmental Engineer Fayetteville Regional Office. Please find attached a copy of our Compliance Inspection Report for your information. It is our opinion that this facility is operating in compliance with permit requirements and that management of this facility is being satisfactorly performed. SECTION I Administration and Program Management S U N/A 1. Is a copy of the issued permit on hand for reference? _X_ 2. Is/are the application(s) site(s) fenced, posted, or otherwise protected to exclude public health problems? X 3. Are periodic reports submitted as required by the permit? X 4. Has an emergency procedures plan been developed and implemented to contend with spills of liquid, dried, or filter press sludges? X 5. Does the Permittee conduct their own analysis of the applied sludge? X 6. Has the analysis of the applied sludge been conducted on a continuous and planned basis? X 7. Does the Permittee or sludge producer have sufficient sludge holding capacity to contend with a no applica- tion situation? X 8. Are application sites identified with estimated or actual size in acres per site? X _ SECTION II Field Site Management S U N/A 1. Is a crop grown and harvested on the site(s)? (Animal Feed) X 2. Are soils analysis being conducted annually and soil conditions being maintained in accordance with the permit requirements? X 3. Is sludge analyzed annually? X 4. Are monitoring wells utilized and being sampled to monitor groundwaters? X 5. Is pH of site being maintained in accordance with permit? X 6. Is there any evidence of endemic crop, soil diseases, or pests? X 7. Are appropriate buffer zones being maintained in accordance with the issued permit? X 8. Are slope limitations being observed in accordance with the issued permit? X 9. Is there any evidence of runoff from the application sites? X 10. What is the general condition of this system (ie, nuisance conditions present)? X 11. Are there any special land or crop requirements; ie, row crops, strip contouring, etc.? (If yes, give details in Comments Section.) X 12. Is freeboard in wastewater lagoon adequate? X Records and Records Management 1. Are records being maintained of all analysis made? X 2. Are records maintained to indicate pounds/acre being added to the defined plots a/o site(s)? X 3. Are applications being made to privately owned farm land?
