information request, dated March 7, 2008, as it regards the renewal of Permit #WQ0000672.
Only one item was addressed in your request for additional information. To that end, I have
enclosed two copies of soil maps for each land application site along with two -copies of the soil
legend for .Robeson County. In the initial application package, each 'site has a separate "Site
Summary" page. On that page is listed the predominant mapping, unit for each field. The listings
on those site summaries are correct. When I prepared the renewal application, I reviewed each
soil map in order to.determine the predominant map unit. However, I' neglected to provide copies
of those maps to DWQ/LAU.

The Land ApplicationXnit has, completed a:thorougli technical-, review of -the ,subject permit
renewal application package. At this time; additional information is required before we may
continue our..review. Please address the items listed below no later than April 7, 2008.
1. County Soil Survey Map (or equivalent) clearly indicating the location and name of
all land application sites to confirm the correct soil -mapping units. The map must be
legible and to a scale where the sites can be easily identified and differentiated from
adjacent sites. The predominant soil=mapping units must be shown.for each site. The
map must include the mapping unit legend, the County name, the Applicants name,
and the permit number. The Permittee must note in the narrative and provide
documentation supported by a licensed soil scientist for approval of any soil -mapping
unit submitted in this form that has not been -previously approved and differs from
that shown in the County Soil Survey.
Please reference the subject application number when providing the requested information.
Two copies of all revised information should be submitted to my attention at the address below.

Thanks for the notification. This will suffice and a copy will be placed in the file for City of
Lumberton. How many more days will Synagro be hauling sludge from the City of Lumberton WWTP
and which fields, besides Thomas Maynor, will be utilized during this event?

Please consider this a formal notification of Synagro's intention to land apply residuals from the City of
Lumberton's WWTP onto sites owned by Thomas Maynor II. This is the first land application event to these
fields, as the site was addedto the program last year.
This notification is in accordance with the City of Lumberton's Land Application of Residual Solids Permit
Number WQ0000672, issued by NC DENR/DWQ. Section -1. 10. states "...the appropriate local
governmental official (i.e., county manager) shall be notified at least 24 hours prior to the initial residuals
land application event on any new site so that they will be aware that residuals land application activities have
commenced on the site". Additionally, Section I.11. states, "the Aquifer Protection Section of the appropriate
Division's regional office shall be notified at least 24 hours prior to the initial residuals land application event
on any new land application site".
The fields are listed, on the current "Attachment B" for the permit referenced above, as "NC-RO-16-20" and
"NC-RO-16-21 ". They are located (just) northwest of the intersection of Buie-Philadelphus Road (listed as
"Mt. Zion Church Road" on "Google Maps") and Oak Grove Church Road and about 6 miles northeast of
Pembroke. -
