Martin, Governor S.Thomas Rhodes, Secretary Mr. Christopher Norris 1924 Bivins Road Durham, North Carolina 27712 SUBJECT: Permit No. NC0058220 Authorization to Construct Norris Residence Single Family Residence Wastewater Treatment Family Durham County Dear Mr. Norris: A letter of request for Authorization to Construct was received March 8, 1985, by the Division and final plans and specifications for the subject project have been reviewed and found to be satisfactory. Authorization is hereby granted for the construction of a 600 GPD wastewater treatment facility consisting of a 1,200 gallon capacity septic tank, a 270 square foot surface sand filter with distribution box, a 27 gallon chlorine contact tank with tablet type chlorinator and a five (5) step cascade aerator to serve the 4-bedroom Norris Residence located at Winders Lane in Durham County. This Authorization to Construct is issued in accordance with Part III para- graph C of NPDES Permit No. NC0058220 issued May 6, 1985, and shall be subject to revocation unless the wastewater treatment facilities are constructed in accordance with the conditions and limitations specified in Permit No. NC0058220. The sludge generated from these treatment facilitis must be diposed of in accordance with G.S. 143-215.1 and in a manner approvable by the North Carolina Division of Environmental Management. The Raleigh Regional Office, telephone No. 919/733-2314 shall be notified at least twenty-four (24) hours in advance of the backfilling of the installed subsurface filter system so that an in-place inspection can be made of said system prior to backfilling. Such notification to the Regional Supervisor shall be made during the normal office hours from 8:00 A.M. until 5:00 P.M. on Monday through Friday, excluding State Holidays. In even the facilities fail to perform satisfactorily in meeting:i,ts NPDES permit effluent limits, the Permittee shall take such immediate corrective action as may be required by this Division, including the construction of additional wastewater treatment and disposal facilities. Per-raft No. NC0058220 Page Two The sand media of the subsurface filter must comply with the Division's sand specifications and must be analyzed and approved by this Division either by direct sampling or by acquisition of filter sand from a dealer who is currently certified by the Division as an acceptable source. Septic tank must be pumped once a year and the filters must be rehabilitated as needed but should be inspected no less than once every three (3) years of operation. One (1) set of approved plans and specifications is being forwarded to you.
