Single Family Residence
Wastewater Treatment Facility
Forsyth County
Dear Mr. Crim:
A letter of request for Authorization to Construct was received January
23, 1984, by the Division and final plans and specifications for the subject
project have been reviewed and found to be satisfactory. Authorization is
hereby granted for the construction of a 0.00045 MGD wastewater treatment faci-
lity consisting of an existing 900 gallon capacity septic tank, an existing 270
square feet subsurface sand filter, a new chlorine contact tank with tablet
type chlorinator, a new cascade-type aerator and associated piping and appur-
tenances to serve the Richard Crim residence in Forsyth County.
This Authorization to Construct is issued in accordance with Part III
paragraph C of NPDES Permit No. NC0057983 issued January 18, 1984, and shall
be subject to revocation unless the wastewater treatment facilities are con-
structed in accordance with the conditions and limitations specified in Permit
No. NC0057983.
The sludge generated from these treatment facilities must be disposed of
in accordance with General Statute 143-215.1 and in a manner approvable by the
North Carolina Division of Environmental Management.
The Winston-Salem Regional Office, telephone number 919/761-2351 shall be
notified at least twenty-four (24) hours in advance of operation of the installed
subsurface filter system so that an in-place inspection can be made of said
system. Such notification to the Regional Supervisor shall be made during the
normal office hours, from 8:00 A.M. until 5:00 P.M. on Monday through Friday,
excluding State Holidays.
In event the faciliites fail to perform satisfactorily in meeting it's NPDES
permit- effluent limits; Richard Crim shall take such immediate corrective action
as may be required by this Division, including the construction of additional
wastewater treatment and disposal facilities such as increasing the surface area
of the subsurface sand filter.

Authorization to Construct
The sand media of the subsurface filter once replaced must comply with the
Division's sand specifications and must be analyzed and approved by this Division
either by direct sampling or by acquisition of filter sand from a dealer who is
currently certified by the Division as an acceptable source.
