Dear Permittee:
The subject permit expires on July 31, 1997. North Carolina General Statute 143.215.1(c) requires that an application for
permit renewal be filed in a timely manner. A prior notice of renewal was sent for this site on February 7, 1997. To date,
no response has been received by the Division. If continuation of the permit is desired, submit the following information
prior to May 31,1997:
1. A letter requesting the renewal.
2. The completed invoice form(copy attached).
3. A renewal fee of$240. The check should be made payable to the North Carolina Department of Environment,
Health, and Natural Resources which may be abbreviated as NCDEHNR.
Failure to request renewal or permit rescission prior to May 31,1997 will result in a civil assessment of at least$250.00.
Larger penalties may be assessed depending upon the delinquency of the request.
If operation of a discharge or waste treatment facility is to occur after the expiration date of the permit, or if
continuation of the permit is desired,it must not be allowed to expire. Operation of the waste treatment works or
continuation of a discharge after the expiration date would constitute a violation of NCGS 143-215.1 and could result in
assessment of civil penalties up to $10,000 per day.
