Dear Mr. Myers:
On October 25, 2005, Jenifer Carter of this office performed a General Permit Inspection on the
wastewater treatment system at the abovementioned address in Davie County,North Carolina. Mrs.
Myers was also present for the inspection. This type of inspection consists of two basic parts: a review of
self-monitoring data, and an on-site inspection of the facility and its discharge.
You hold a General Permit to discharge domestic wastewater from a single family residence under
the National Pollutant Discharge Elimination System (NPDES). You are found to be in compliance with
your General Permit No. NCG550244. There was no evidence of wastewater discharge at the effluent
pipe. Please refer to your peiuiit for monitoring requirements. If you notice any future discharge, please
monitor accordingly the effluent characteristics listed in the permit.
Our files have been updated to show that your septic tank was pumped out on October 29, 2004.
All system components appeared to be functioning properly at this time. If you have any questions
