5. Directions to Site:
Take Hwy 109 south out of Winston-Salem and then turn left
onto Old Greensboro Road (SR 1756) at Ledford Middle School.
The Lauer SFR is located on the left 0 . 9 mile east of Mock
Road (SR 1755) . The house is not visible from the road.
Two large grey steel gates are on either side of the
driveway.

The lot consists of approximately 5 acres and contains
adequate space for upgrading.
8 . Topography (relationship to flood plain included) :
The house is located on a narrow ridge sixty feet above the
flood plain and 100 yards from Abbotts Creek.

9. Location of nearest dwelling: There are no dwellings within
a 1000 ft. Please note that Mr. Gary McClellan holds NPDES
permit No. NC0057568 (not built) for discharge into Abbotts
Creek from an adjoining lot just downstream from Dr. Lauer.
10. Receiving stream or affected surface waters: Abbotts Creek

c. Describe receiving stream features and pertinent
downstream uses: The drinking water reservoir, Lake
Thom-A-Lex is located approximately 5 miles downstream.
The Willow Creek WWTP (NC0034452 , 0. 08 MGD) discharges
into Abbotts Creek 1000 ft. above Mr. Lauer's outfall.
PART II - DESCRIPTION OF DISCHARGE AND TREATMENT WORKS

existing) : The existing system consi2ts of a 1, 000 gallon
septic tank, distribution box, 522 ft primary subsurface
sandfilter, 260 ft secondary subsurface sandfilter, tablet
chlorinator, 800 gallon contact tank and discharge line to
Abbotts Creek.
5. Sludge handling and disposal scheme: Sludge should be
removed from the septic tank by licensed haulers for
disposal in the nearest municipal WWTP.

PART IV - EVALUATION AND RECOMMENDATIONS
The dual subsurface sandfilter system for Dr. Lauer's
residence was installed on 6-17-88. Observations of the effluent
pipe on 9/16/92 revealed that the system was functioning
reissuance.
