form you can complete and return so that we can transfer the Certificate of Coverage (CoC)to you.
I've also copied Ron Boone in our Winston-Salem Regional Office on this message. Ron is the inspector for Forsyth
County, so he will likely be the person to visit your system.
Ron—Mrs.Sexton is a new homeowner.She and her husband received little info from the previous owner about the
system and its maintenance requirements, etc. Please contact her[336-705-8424] and plan a site visit as your schedule
permits.They'd be grateful for any technical advice the WSRO can offer.
