Dear Mrs. Haddock:
The NC Division of Water Resources, Washington Regional Office and NC Operator
Certification Program staff met with you at: Washington Regional Office on October 14,
2024, at 10:30 am to discuss the discharge of wastewater and improper irrigation practices
originating at the Keene Farm (AWS740022).
The concerns noted during the Show Cause Meeting included: The discharge of wastewater,
improper application of waste, a high lagoon level, failure to notify the Division and the
cleanup measures after the discharge.
Based on the findings of this meeting, the DWR Washington Regional Office will not
recommend any additional disciplinary actions to the Water Pollution Control System
Operators Certification Commission (WPCSOCC) at this time. Please note that all
information pertaining to this show cause meeting can be used as supporting evidence for
any future disciplinary actions.
As a licensed animal waste operator in North Carolina, this Office strongly recommends you
review, in addition to all other applicable rules and regulations, the requirements set forth
in:
