In regard to our phone conversation on January 5, 1993 , I am -
sending this letter to aid you in your Mother, Ms. Pearl Goforths :- : , ;,
permit renewal process. As mentioned on the phone, Pearl Goforths :,'
situation is unique. In 1988 she was over charged for her renewal - ==-:c
application. Fortunately I have worked it out so this renewal --i
application fee will only be $30 . 00 to reflect the difference of the
$150. 00 she was charged in 1988 and the normally required $120. 00. In
order to fulfill the renewal application requirements you will need to
send the following:
1 . A letter requesting renewal of Permit Number NC0056707 .
2 . A check for $30. 00, made payable to North Carolina
Department of Environment, Health, and Natural Resources,
this can be abbreviated DEHNR. Also, it is a good idea to
write the permit number on the check.
3 . Three signed copies of the applications, Short Form D.

Enclo.Ged you will find four copies of the application which I
have filled out for you. All you have to do is sign and date the
reverse side of all four copies. As mentioned above, three of these
get sent back to this Department, the fourth one is for you to keep
with your records . It is important that you get the mentioned items
back to me before the end of the month so I can see to it that
