Division of Water Resources’ Central and Regional staff have reviewed the application package
received on May 23, 2024. However, the Division requires additional information before completing our
review. Please address the items on the attached pages no later than the close of business on September 11,
2024.
Please be aware that the Applicant is responsible for meeting all requirements set forth in North
Carolina rules and regulations. The Applicant is also responsible for any oversights that occur during the
review of the subject application package. The Division may return the application as incomplete pursuant
to 15A NCAC 02T .0107(e)(2) if any omissions are made when responding to the outstanding items in
Sections A through P or the Applicant fails to provide the additional information on or before the above-
requested date.

L. Attachment I – Compliance Schedules:
1. Condition I.1 of the Final Permit issued on May 21, 2019 (included as an attachment), required the
Permittee to submit documentation to the Division within 30 days of permit issuance that the pipe
to the River Dunes Disposal Site has either been permanently plugged or physically disconnected
such that no water can either be conveyed to or from the River Dunes Disposal Site.
Ms. Sarah A. Toppen of the Washington Regional Office (WaRO) conducted a site visit on August
2, 2024, reviewed the submitted application package, and confirmed that the Applicant has only
temporarily capped the pipe.
The Division has not received any physical and/or electronic documentation since the previous
permit issuance to satisfy this Compliance Schedule. Please provide documentation that fulfills
this Compliance Schedule. [15A NCAC .0108(b)(1)(B)]
2. Condition I.2 of the Final Permit issued on May 21, 2019, required the Permittee to submit a Site
Map to the Division within 180 days of permit issuance including the following:
a. Legend, north arrow, and scale.
b. Topographic contour intervals not exceeding 10 feet or 25 percent of total site relief.
c. All habitable residences or places of assembly within 500 feet of the irrigation area.
d. Location of all wells, streams (ephemeral, intermittent, and perennial), springs, lakes, ponds,
ditches, and other surface drainage features within 500 feet of the irrigation area.
e. Location and identification of each monitoring well.
f. Latitude and longitude coordinates of each monitoring well to the sixth decimal degree.
g. Location and identification of all wastewater treatment and storage units.
h. The perimeter of all irrigation areas with field names as listed in Attachment B.
i. Location and ownership of property boundaries within 500 feet of the irrigation area, including
rights-of-way and easements.
j. Latitude and longitude coordinates of the established horizontal control monument to the sixth
decimal degree.
k. Elevation of the top of the well casing (i.e., measuring point) relative to a common datum.
l. Depth of water below the measuring point.
m. Delineation of the compliance and review boundaries.
n. Distance measurements verifying all setbacks.
o. Stormwater drainage controls.
p. 100-year floodplain (if present).
q. The date the map is prepared or revised.
The Division has not received any physical and/or electronic documentation since the previous
permit issuance to satisfy this Compliance Schedule. Please provide a Site Map that fulfills this

3. Condition I.3 of the Final Permit issued on May 21, 2019, required the Permittee to submit a
detailed closure plan for the storage basin at the River Dunes Disposal Site within one year of
permit issuance. The closure plan shall utilize the Division’s Guidelines for the Closure of
Permitted Wastewater Ponds and Lagoons (included as an attachment) and include the following:
a. Piping/pumping modifications to ensure wastewater can no longer flow to the infiltration
basins.
b. Abandonment procedures necessary to address the existing synthetic liner and vegetation
growing around the basin that could pose a safety risk.
c. Abandonment timeline.
The Division has not received any physical and/or electronic documentation since the previous
permit issuance to satisfy this Compliance Schedule. Please provide documentation that fulfills
