This modification reduces the effluent monitoring frequencies for Biochemical Oxygen Demand
(BOD5), Total Suspended Solids (TSS), Ammonia Nitrogen (NH3-N) and Fecal Coliform from daily
(5/week) to 2/week monitoring. The modification was based upon an evaluation of three years of
facility effluent monitoring data. The facility’s performance satisfies the criteria established in the
“DWQ Guidance Regarding the Reduction of Monitoring Frequencies in NPDES Permits for
Exceptionally Performing Facilities” as approved by the Director of the Division of Water Quality on
October 22, 2012, and justifies reduced monitoring for these parameters. The modified monitoring
frequencies will become effective on February 1, 2013.

The Infrastructure Finance Section performed a Final inspection for the state and federally
funded portion of subject project on Monday May 20, 2013. Interim inspections were conducted
throughout the project beginning after the Preconstruction meeting of August 10, 2011 and generally
occurring on the 2nd Thursday of each month through November 2012. The progress meetings were
conducted in the conference room at the facility located on NC Highway 55. Contract substantial
completion date was March 1, 2013.

I. FINAL INSPECTION - Action Items and Comments
The NC Infrastructure Finance Section (IFS) requests a response below from either the Owner or
Engineer, as applicable. Item(s) may be addressed via a statement in a cover letter or by
submitting the requested information. Except for Change Order(s), it is suggested all required
information be submitted at one time. If forms are needed to comply with items below, refer to
IFS web site ifs.nc.gov.
Upon completion of review and edits by Owner, Engineer and Contractor of the Record
Drawings, provide a set of half sized Final Record Drawings to the Infrastructure Finance
Section.
Provide to IFS a list and status of Manufacturer’s Operation & Maintenance Manuals and
attest that list is complete.
Engineer needs to confirm in writing to IFS when all test reports and documentation including
manufacturer’s and equipment startup reports have been provided to Owner.
Engineer needs to confirm in writing to IFS when all Inspector’s Logs, including storm water
reports and documentation have been provided to Owner.
Owner requested Engineer provide complete log of Requests for Information (RFIs) be
provided. Engineer needs to confirm in writing to IFS when this is accomplished.
Provide to IFS a list of all warranty information documentation for the contract. Confirm that
the information has been provided to all parties including PC Construction, that is, all parties
should be aware of warranty date(s).
Provide IFS copy of Contractors Release of Liens.
Provide IFS copy of Contractors Consent of Surety.
Provide IFS complete list of spare parts and statement that all spare parts have been received
by Durham County.
Provide complete list of all training sessions and statement that all training has been
satisfactorily provided.
11) Change Orders through no. 6 have been processed by IFS. It is understood pending
completion of punch list items and resolution of item 12) below there will be a
Change Order #7 and possibly #8. Submit change order(s) necessary to match the amount of
the final reimbursement request and submit Final Reimbursement request. Please note you
must provide printed names and titles of signatories on original change order.
12) An investigation is in progress on the structural and leak integrity of the wall between the
pump room and the sludge tanks. Upon resolution of this matter, provide IFS with summary
description of problem and resolution and if reimbursement of any costs will be sought. IFS
cannot participate in funding of this matter until or unless Owner provides documentation
required in the change order checklist and the work is determined eligible.
13) Please note that Owner is responsible for seeking project related Sales Tax reimbursement
through the NC Dept, of Revenue.
Submit Owners Certification of Completion when appropriate.
A Partial Engineer’s Certification has been submitted. When appropriate submit Engineer's
(Final) Certification.
Submit final reimbursement request.
For the record, Davis Bacon records were reviewed by IFS during the project and it is noted
that project records appeared to be in compliance. Owner is advised to maintain certified
payroll records, however the requirement to conduct Davis Bacon interviews was rescinded in
November 2012.
Provide final MBE/WBE utilization report.
19) IFS believes that subject to satisfactorily resolution of above items, the project is complete.

As we discussed by phone this morning, the Triangle Wastewater Treatment Plant and Collection system weathered the
storm without any know spills. Prior to the storm we activated our IWERP (Inclement Weather Emergency Response
However to weather the storm, we did use almost all of our countermeasures, including:
Diverting peak influent flow to almost fill Train 3.
Filling and starting a third clarifier.
Running all UV at 100%.
Operating the pressure sewer pump stations at Scott King and Fairfield manually in a dual pump mode. To
maximize pumping rate, we alternated operation of each station.
Operating our largest pump station Stirrup Iron manually with peak flows of 3000 gpm.
Operating our influent pump station at high flowrates to minimize surcharges in the collection systems .
We also received some help from our customers, as follows:
At the Slater Road pump station, we were at a high water alarm level. The Town of Cary has a nearby sewer
system, and it had capacity. The Town approved our pumping to their sewer system if we were faced with
overflow. We set up a portable pumping system, but never had to use it as the flow subsided before the pump
station reached its maximum capacity. The Town of Cary monitored their system during this portion of the
event. We appreciate the assistance from the Town.
Our largest flow industrial user, Cree, has a large equalization basin, and at approximately 6:30 p.m. we asked
them to stop their flow for as long as possible. They stopped their flow for about 2 hours. This gave Stirrup Iron
pump station some relief during the peak flow period.
Based on our current conditions and the stormy hot season ahead, we have chosen to begin wasting sludge to Train 3
and will begin bringing it online in the next few days. Bringing a train online smoothly is a difficult operation and we will
make every effort to do so without violating any effluent standards. If we have any problems we will let you know as
soon as possible.

Change order #6 is for 15 items including 3 credits and 12 adders. Adders included
modifications to conduit runs and materials, modification to OH doors and structure,
additional rebar in pump room slab, DIP and centrifuge pipe and connection
modifications, and miscellaneous electrical. Credits included rerouting of fuel lines to
generator, rerouting of conduit, and adjustment to I/O quantities from CO#4. There was
a request for 42 additional days to be added to substantial completion changing that date
from Oct. 26 to Dec 7, 2012.

The Infrastructure Finance Section of the North Carolina Division of Water Quality has
concluded review of the information received in March 2013 in support of subject change order.
Accordingly, the following determinations have been made:
* Total Loan amount is limited to $11,589,400.
