The inspection consisted of reviewing data for NPDES #NC0026051. The staff is congratulated for doing
a good job of maintaining the laboratory certification program. The laboratory is spacious and well
equipped. All facilities and equipment appear to be well maintained. Records are well kept and were
easily accessible to the inspector. The data that was reviewed for the most part looked good. Some
further quality control procedures need to be implemented.
Comment: For all benchsheets, it is recommended that the lab indicate which edition of the method is
being referenced. This will aid the analysts, when method changes are promulgated by EPA, to
maintain adequate documentation of their data.
A). Comment: The meter appeared to be in good shape, although the probe appeared a little dirty and
the sponge in the storage well was found to be quite dirty indicating the probe is not being properly rinsed
and maintained. The analyst changed out the sponge during the inspection.
Comment: The laboratory is currently using “Ultraviolet” purification, thus has no immediate need to
measure residual chlorine at its effluent stream. Even though no effluent measurements are being
performed, the laboratory had performed the annual (5) point calibration verification check on the

Requirement: “Rinse DO electrode between determinations to prevent cross-contamination of samples
and to insure overall good laboratory practice. Ref: Standard Methods 18th Edition, Method 5210, page 5-
5, 4. f. 2)
This laboratory was inspected to verify its compliance with the requirements of 15A NCAC 2H .0800 for
the analysis of environmental samples.
B). Comment: The laboratory is including the blank in the analytical curve

instrument. There was no residual chlorine data for review since none is being reported on the DMR
reports.
C). Comment: The laboratory is not checking the Automatic Temperature Compensation device
annually. The new policy and the proper procedures are stated below.

Requirement: The laboratory must not include the blank or zero as a point in the analytical curve.
Analytical curve linearity must be calculated between the low and high standards of the analytical curve
only. As a minimum, measure three different dilution's of the standard when an analysis is initiated. A
blank does not contain a measurable amount of standard; therefore "zero or the origin” cannot be
considered in the calibration curve. Ref: Standard Methods, 18 Edition - Method 1020 B Pg. 1-5.
Note: Even though no residual chlorine values are being measured or submitted on the DMR reports
the laboratory must still conform to all parameters listed on the NC Certificate.
C. Comment: The laboratory is currently using a NIST certified thermometer for calibrating all of the
temperature measuring devices. Unfortunately, and even though documentation showed that this
thermometer was “recalibrated” by Barnstead/ERTCO laboratory in 2003, the inspector found this
thermometer to be damaged. After very close inspection of the thermometer it was discovered that the
mercury had separated, making it unacceptable for any accurate temperature measurements.
Note: The inspector advised the laboratory that its current NIST calibrating thermometer was not
adequate for calibrating any temperature measuring device. The analyst assured the inspector that a
new thermometer would be purchased and documentation submitted to the inspector upon its arrival.
In the meantime the laboratory will have its devices verified with a NIST traceable thermometer before
any additional effluent measurements are taken.
Requirement: (New Policy) Temperature Sensor Check (pH meter/ DO meter/Conductivity) (Check
every 12 months)
To check the temperature sensor read the temperature of the meter against a NIST thermometer and record
the two temperatures. It is recommended that the correction factor be no more than 1 degree. The
correction factor must be less than 3° C. In the documentation include the serial number of the NIST
thermometer or traceable thermometer that was used in the comparison. Also document any correction that
applies on both the instrument and on a separate sheet to be filed.
Requirement: The temperature sensor must be calibrated annually even if it is not being used to
report temperature.
Requirement: (New Policy) Automatic Temperature Compensator Check (ATC) for pH meter and
Conductivity meter: (Check every 12 months)
1. Take a buffer or standard solution and cool to below 10°C and read the temperature. Analyze the buffer
or standard solution at that temperature. Document the temperature and the measured pH or standard
solution value obtained on the log sheet.
2. Raise the temperature of the buffer or standard solution to approximately 25°C. Document the
temperature and the measured pH or standard solution value on the log sheet.
3. Raise the temperature of the buffer or standard solution to approximately 35°C. Document the
temperature and the measured pH or standard solution value on the log sheet.
4. Repeat with additional temperature adjustments and pH or standard solution readings as necessary to
ensure the temperature usually measured for samples has been bracketed. As the temperature increases
or decreases the value of the buffer or standard solution should remain approximately the same. (Should
not vary by more than ± 0.1 s.u. from the true value of the pH buffer or 10% for the Conductivity standards).

A paper trail was conducted at this facility. This consisted of comparing laboratory contract data and
raw data of all parameters with quarterly DMR reports from March, August and November, 2005 and
some data transcription errors were detected. For March 27 through March 31, 2005, the
Temperature values were transcribed as Dissolved Oxygen values on the DMR. The inspector
advised the laboratory that it was required to file an amended DMR with the regional DWQ Central
office stating the data corrections within 30 days and a copy be submitted to the inspector with the
inspection reply.

The inspector would like to commend the analysts in assisting in the data review and
inspection process. Correcting the above cited comments and implementing the
requirements/recommendations will help this lab to produce quality data and meet certification

The aquatic toxicity test using 24-hour composite samples of effluent discharged from Durham
Co.-Triangle WWTP has been completed. Durham Co.-Triangle WWTP has an effluent
discharge permitted at 6.0 million gallons per day (MOD) entering Northeast Creek (7Q10 of
0.00 CFS). Jennie Atkins collected whole effluent samples on March 7, 2006 for use in a
chronic Ceriodaphnia dubia pass/fail toxicity test. The test using these samples resulted in a
pass, but these results should not be used for compliance monitoring purposes. The test was
deemed “invalid” due to not meeting quality assurance criteria, as no second sample was
collected. The following toxicity assessment is based on test data collected through 5 days (2
broods) and exposure to a single effluent sample. Toxicity test information follows.

Some of the flow monitoring frequency requirements are not clear. Several of the
permits have frequencies of “Each composite event/monthly average”. However, the
Special Condition #2 describing that requirement varies for each one. The Week IUP
has a good explanantion for this frequecy and it is assumed that this is what is
intended for the lUPs with this requirement. Next time this lUPs are modified or
renewed, please clarify this requirement.
The frequency of “Continous” is used in some of the other lUPs. “Continous” more
accuratley describes a monitoring method. It is assumed that what is intended is that
the User record the flow meter reading each day and submit a monthly summary.
Next time these lUPs are modified or renewed, please clarify these requirements.

.0903(b)(34) and the definition of Significant Industrial User. The allocation table maintained in
