On October 15, 2024, the North Carolina Division of Water Resources Underground Injection Control Program
(UIC Program) received your request to rescind permit W10500242 for injection activities at the United Parcel
Service — Rocky Mount located at 1108 Instrument Drive, Rocky Mount, NC 27801.
The UIC Program has reviewed your request and has determined that the information provided to date is
sufficient for completing the monitoring and compliance reporting requirements of the permit. Therefore, the
permit has been closed.
Please note that any other compliance monitoring or reporting for any other regulatory agency shall continue
as required.
If you wish to conduct future injection operations, you must first apply for and receive a new injection well
permit. The injection well permit application, well construction/abandonment forms, and other UIC-related
forms can be downloaded from the UIC Program's website at (https://www.deg.nc.gov/about/divisions/water-
resources/groundwater-resources/forms-permit-application-tracking). Operating an injection well without a
valid permit could result in a civil penalty of up to $25,000 per day.

In lieu of a FPE, do you have a copy of the most recent groundwater data from the site? The most recent monitoring
report for the site? Or something like that? We just need to see the groundwater data pre- and post -injection to
determine if there are any residual injectants at the site.

Thank you for providing the GW-30 forms, the NFA, and NRP. I will need review (but not limited to) the following
items since they were not included in the letter:
1. A completed GW-68 form. I did not see one in the attachment that you sent, please let me know if I missed
it.
2. A Final Project Evaluation or equivalent that contains analytical data pre/post injection. Sodium persulfate
was permitted for this injection.
