Dear Mr. Keinmaier,
Division of Water Resources' Central and Regional staff have reviewed the application package
received on March 26, 2024. However, the Division requires additional information before completing our
review. Please address the items on the attached pages no later than the close of business on July 22, 2024.
Please be aware that the Applicant is responsible for meeting all requirements set forth in North
Carolina rules and regulations. The Applicant is also responsible for any oversights that occur during the
review of the subject application package. The Division may return the application as incomplete pursuant
to 15A NCAC 02T .0107(e)(2) if any omissions are made when responding to the outstanding items in
Sections A through M or the Applicant fails to provide the additional information on or before the above -
requested date.
Please reference the subject application number when providing the requested information. The
Applicant shall sign, seal, and date (where applicable) all revised and/or additional documentation and
submit an electronic response to my attention via the Non -Discharge online portal.

A. Overall:
1. Regional Office Staff Member Melanie Kemp noted the following during her review of the
application:
It should be noted that the current permit description and as -built drawings do not clearly indicate
which facilities are connected to the existing septic tanks. Based on original as -built drawings, the
following facilities flow into the existing WWTF:
• 2 x Septic Tanks (4,000 gallons and 2,000 gallons):
o 3 x 3-bedroom homes
o Hatchery staff building
• Septic Tank 3 (5,000 gallons):
o Wildlife Education Center
The existing septic tanks will be removed and replaced with the following:
• Septic Tank 1 (4,000-gallon tank; expected 1,655 gpd flow):
o Hatchery staff bathrooms (new building)
o 3 x 3-bedroom houses (existing buildings)
0 Septic Tank 2 (3,500/4,000-gallon tank; expected 1,950 gpd flow):
o Public Restroom (new building)
B. Cover Letter:
1. Section A of Form: WWIS 06-16 requires a cover letter be submitted that shall "List all items
included in the application package, as well as a brief description of the requested permitting
action.". No cover letter describing the scope of the project was submitted. Please submit this
document.
C. Application (Form: VV IS 06-16):
1. Regional Office Staff Member Melanie Kemp noted the following during her review of the
application for the following sections:
a. IV.7: Design flow table is not filled out correctly. Previous approved flow rate was 7,500 but
based on calculations provided in application, updated flow is 3,605, as determined by 02T
.0114 rules. Please clarify how flow calculations were determined and complete the table based
on the last page provided with the application package.
b. V.1: There are no engineering calculations provided to substantiate the modifications.
c. V.9: A box needs to be checked here.
d. V.11: Facility design criteria need to be provided.
e. VIII.3: Setback information needs to be provided or marked as N/A where applicable.
Please address the above and ensure that the application is filled out completely.

D. Property Ownership Documentation:
1. Per 15A NCAC 02T .0504(f), the Applicant shall demonstrate they are the owner of all property
containing the wastewater treatment, storage and irrigation facilities. This documentation may
include:
a. Legal documentation of ownership (i.e., GIS, deed or article of incorporation), or
b. Written notarized intent to purchase agreement signed by both parties with a plat or survey
map, or
c. Written notarized lease agreement that specifically indicates the intended use of the property and has
been signed by both parties, as well as a plat or survey map. Lease agreements shall adhere to the
requirements of 15A NCAC 02L .0107
No Property Ownership Documentation was included with the application. Please provide this
documentation along with any agreements, easements, setback waivers, etc. that have a direct
impact on the wastewater treatment, conveyance, storage, and irrigation facilities.
E. Water Balance:
1. Per 15A NCAC 02T .0504(k) and the Water Balance Calculation Policy, submit a water balance
that has been signed, sealed and dated by a qualified professional. Regional Office Staff Member
Melanie Kemp noted that a water balance will need to be provided due to a change in storage
capacity. Please submit an up-to-date water balance.
F. Engineering Plans:
1. Regional Office Staff Member Melanie Kemp noted the following during her review of the facility:
a. CE-101: Plans show 2 x 5,000 gallon septic tanks next to the old visitors center, but existing
permits and as -built drawings indicate one — please clarify. Plans also label " 4,000 gallon
septic tanks" — are there multiple tanks or just the one indicated on the existing permit? Is the
2,000-gallon septic tank, noted on the current permit, identified on this drawing?
b. C-220: Plans show contradictory sizes of septic tanks (1 x 4,000 gallon and 1 x 3,500 gallon
on map but 2 x 4,000 gallon on elevation profile).
Please make any necessary corrections/clarifications to the above concerns.
G. Specifications:
1. Regional Office Staff Member Melanie Kemp noted that there were no specifications provided
detailing the "Proposed Effluent Recirculation Pump Station" that has been detailed on plan set C-
220. Additionally, she noted that no septic tank specifications have been provided. Per Form:
WWIS 06-16, specifications shall represent a completed design. Please update the specifications
package to include all designed units.
H. Engineering Calculations:
A set of engineering calculations has not been submitted. Please submit a set of engineering
calculations pursuant to the requirements outlined within 15A NCAC 02T .0504(c)(3) and Section
K of Form: WWIS 06-16.

I. Site May:
1. A site map has not been submitted. Please submit a site map pursuant to the requirements outlined
within 15A NCAC 02T .0504(d) and Section L of Form: WWIS 06-16.
J. Power Reliability Plan:
1. A power reliability plan has not been submitted. Please submit a power reliability plan pursuant to
Section M of Form: WWIS 06-16.
K. Operation & Maintenance Plan:
1. No updated operation and maintenance plan has been included with the application. Please provide
an up-to-date operation and maintenance plan pursuant to 15A NCAC 02T .0507(a) and Section N
of Form: WWIS 06-16.
L. Residuals Management Plan:
Per Section O. of Form: SFRWWIS 06-16, any application packages with new, expanding or
replacement wastewater treatment systems shall include a Residuals Management Plan. Please
submit an updated Residuals Management Plan.
M. Additional Documentation:
➢ Existing Permit:
1. The existing permit has not been included. Please submit the previous permit with the updated
application package.
➢ Final Environmental Document:
1. Section P. of Form: SFRWWIS 06-16 states that any application package using public monies
or lands subject to the North Carolina Environmental Policy Act shall submit one copy of the
environmental assessment and a copy of the final environmental document (i.e., Finding of No
Significant Impact or Record of Decision). If either of the above conditions are met then please
submit adequate environmental documentation.
➢ Floodway Regulation Compliance:
1. No floodway regulation compliance documentation was submitted with the application. If any
portion of the wastewater treatment, storage, or irrigation system is located within the 100- year
floodplain, please submit written documentation from all local governing entities that the
facility is in compliance with all local ordinances.
➢ Threatened or Endangered Aquatic Species Documentation:
1. Section P. of Form: SFRWWIS 06-16 requires all packages to include documentation from the
Natural Heritage Program demonstrating the presence or absence of threatened or endangered
aquatic species within the boundary of the wastewater treatment, storage and irrigation
facilities. Please provide this documentation.
