I was out sick yesterday and haven't had a chance to respond.
We discussed the alternative option of filling out a new NOI form with Craftmaster Furniture listed as the
Owner/Operator, and including a copy of something like a utility bill in support.You don't have to submit an
application fee—just fill out the Notice of Intent(NOI)application with updated information and have the
appropriate authority sign it so we have a correct record of ownership for this permit. The NCG18 NOI can be
found here.
Austin can process the Owner Name change for NCG180238 from that. I'll work with him on it.
At the top of the form, be sure to enter the existing permit number and a note that this is for an existing permit
ownership correction.

I can have Kevin Mann sent any of the utility bills, production reports, etc., all reflecting the Craftmaster Plant 6
operation. The building is owned by Hamilton Square (Real Estate arm) but the equipment and the operation is
controlled by Craftmaster. At this point, you can permit it either way, as owned by Hamilton Square or Craftmaster
Furniture, as long as the name of the operation is Craftmaster Furniture Plant 6. Let us know how you want to renew
the permit. If you need additional information, let Kevin or me know. Otherwise let me know when you write the COC
and I will request renewal, as the permit fees have been paid.

Do you all have any kind of documentation/paperwork that shows the company name Craftmaster Furniture is
operating this facility?
The previous supporting documents all have Hamilton Square LLC,so you can understand why we have a dilemma
in changing it without additional documentation to correct/update the current owner name. Unfortunatelywe
cannot just take anyone's word given the previous records on the change provided. Does Kevin have
documentation he can provide? For example, somethingthat shows Craftmaster Furniture is the parent company
of Hamilton Square LLC, along with a request that the permit be revised to be issued in that name instead of how it
was requested in 2018?

All we have is what we submitted when we bought the plant in 2018. Do you want me to submit that again? Will
you take Kevin's word that it is now been put under Craftmaster Furniture. He is the Chief Operating Officer.

This is the link to the permit folder if you look back in December 19 2018,there was Owner Name
Change submitted. They submitted 92 pages to support the change to the Owner name Hamilton
Square LLC.
Regardless of whether that change was a mistake or previous name, if you all want it changed from
Hamilton Square LLC to Craftmaster Furniture, please submit supporting documentation for the
current Owner/Operator entity name (Craftmaster Furniture).

I spoke to David Stout earlier today because he tried to request a renewal COC, and he says the old
Owner name is still coming up.The Facility name is correct now, but the Owner is no longer
supposed to be Hamilton Square LLC (that's the real estate arm of the company). David explained
it's supposed to be Craftmaster Furniture.
I couldn't find the documentation in Laserfiche if he sent you something, so I wasn't sure whether it
got overlooked, or maybe there was some info missing that you still need in order to make that
change.
Please let David know if there's something else you need to make that update to the Owner. Let me
know if you have any other questions. I'll be in the office tomorrow if we need to visit on this one.
