I have made the following changes to the Smithfield Renewal:
D. I have removed Tim Weaver as the backup ORC and added Roy Whitaker in his place.
G. The analysis that was submitted with the renewal was an older analysis from when
Smithfield was adding caustic to their process. That is no longer the case and I have
included the most recent analysis.
Q. I have added the property line and corresponding buffers to field SF03-06. In the past,
we have submitted property line waivers for different jobs, but they were no accepted by
the State. If in the future we are able to obtain and file both waivers, will they be
honored?
R.
1. The field has been split into 6A and 6B. The map has been updated and the LASCA
form has been updated with the new acreages.
2. The map has been updated and the acreage has been updated on the LASC form.
3. The parcel in question is a power station. There is no place of assembly. A
property line buffer has been added and the acreage has been updated on the
LASC form.
4. The map for SF08-05 has been included. I must have had a file name issue when
submitting the original.
5. The residence and corresponding buffer has been added to the map. The acreage
has been updated on the LASC form.
6. The area has been added back to the map and the LASC form has been updated.
I have included one PDF that includes the LASC form and all of the map corrections. I will wait
to hear from you about the corrections, and will re-submit the complete package online once
everything looks good.

Division of Water Resources’ Central and Regional Office staff have reviewed the application
package. However, the Division requires additional information before completing our review.
Please address the items in the attached Additional Information Request #1 no later than the close
of business on July 22, 2024.
All revised and/or additional documentation shall be signed, sealed, and dated (where needed), with
an electronic response submitted in a single PDF as an email attachment (document size permitting)
or via the Non-Discharge online portal as “Additional Information”. Please do not resubmit
documentation that has no outstanding comments or revisions (i.e., manufacturer’s information,
product cut sheets, flow reduction request, etc.)
Failure to provide the requested additional information on or before 5:00 PM ET on July 22, 2024,
shall result in your application being returned as incomplete pursuant to 15A NCAC 02T .0107(e)(2).

The above referenced project has been analyzed per your instructions. The analyses were performed in
accordance with the applicable analytical method. Where the laboratory was not responsible for the sampling
stage (refer to the chain of custody) results apply to the sample as received.
The analytical data has been validated using standard quality control measures performed as required by the
analytical method. Quality Assurance, method validations, instrumentation maintenance and calibration for all
parameters (NELAP and non-NELAP) were performed in accordance with guidelines established by the USEPA
(including 40 CFR 136 Method Update Rule May 2021) and NELAC unless otherwise indicated. Any parameter
for which the laboratory is not officially NELAP accredited is indicated by a '~' symbol. These are not included in
the scope because NELAP accreditation is either not available or has not been applied for. Additional
certifications may be held/are available for parameters, where NELAP accreditation is not required or applicable.
A full list of certifications is available upon request.
Certain parameters (chlorine, pH, dissolved oxygen, sulfite...) are required to be analyzed within 15 minutes of
sampling. Usually, but not always, any field parameter analyzed at the laboratory is outside of this holding time.
Refer to sample analysis time for confirmation of holding time compliance.
The results are shown on the attached Report of Analysis(s). Results for solid matrices are reported on an as-
received basis unless otherwise indicated. This report shall not be reproduced except in full and relates only to
the samples included in this report.
