The purpose of the inspection was to ensure compliance with State Stormwater Management Permit SW6180703 prior to the transfer of this permit to the Avery Pond Homeowner's Association. This permit was issued on October 28, 2015. A copy of the Compliance Inspection Report is enclosed for your review. As a result of the inspection and subsequent file review, it has been determined that the facility is not compliant with the conditions of the subject permit and approved plans. The following issues in regards to the operation and maintenance of the stormwater control measures were observed.: 1. Pond 1 - One of the two permanent diversions/conveyance swales (inlets)to the forebay as shown on the approved plans appears to have been eliminated. A swale is installed which is discharging to a wetland instead of to Pond 1. A revised stormwater plan will need to be submitted regarding these modification or the stormwater control measures will need to be installed per the approved plans. 2. Pond 3: One of the two permanent diversions (inlets) to the forebay as shown on the approved plans appears to have been eliminated. A revised stormwater plan will need to be submitted regarding this modification or the stormwater control measure will need to be installed per the approved plans.
