The UIC Program has reviewed your request and has determined that the information provided to date is sufficient for completing the monitoring and compliance reporting requirements of the permit. Therefore, the permit has been closed. Please note that any other compliance monitoring or reporting for any other regulatory agency shall continue as required. If you wish to conduct future injection operations, you must first apply for and receive a new injection well permit. The injection well permit application, well construction/abandonment forms, and other UIC-related forms can be downloaded from the UIC Program's website at (https://www.deg.nc.gov/about/divisions/water- resources/groundwater-resources/forms-permit-application-tracking). Operating an injection well without a valid permit could result in a civil penalty of up to $25,000 per day. Eric, UIC Permit No. W10600071 was issued in March 2011 for injection of Adventus EHC and ZVI to remediate groundwater impacts at the One Hour Cleaners site in Laurinburg.
