respectfully submits to the North Carolina Department of Environmental Quality
(NCDEQ) Underground Injection Control (UIC) Program this Status of Injection
Well(S)/Permit Rescission Request Form requesting to rescind UIC Permit# W10S01030
issued for the Former Guilford Mills Facility (the Site). As requested byyou,the 2027
Monitored Natural Attenuation Groundwater Monitoring Report, dated February 2022, is
included as an attachment.The report summarizes the 2021 MNA groundwater
monitoring events and results, injection well abandonment activities, and includes a
summary of the 2020 groundwater injection activity and historical analytical data for the
Site.
