On September 3, 2024, Gary Hudson of this office performed a records review on the wastewater
treatment system referenced above. There is a Twin Oaks Grocery site, incident # 21289 and a Twin Oaks
Grocery-B site, incident # 30775. Incident 21289 was closed on 11/12/2004. It is our understanding that
incident 30775 has not been in operation since the spring of 2023, after reaching levels of attainment.
