Additional information is required before we may continue our review. Please address and submit the following item(s) within 30 (thirty) days of receipt of this letter. ❑ Operation Type — The operation type marked on the application is 250 Swine Wean -Feeder, but this appears to be for 250 Beef Feeder. Please confirm. ❑ Missing Certification Form — Please have Certified Technical Specialist complete and return the attached Animal Waste Management Plan Certification Form. ❑ Manure Storage — Is there any planned manure storage — wet stack? Dry stack? If so please provide location and design information. ❑ Lease Agreements — A signed land application lease agreement, including expiration date, must be included for any fields in the Nutrient Management Plan not owned by you. Please confirm ownership or provide lease agreements. ❑ Operation & Maintenance Plan — Please provide a site -specific Operation & Maintenance Plan. ❑ OIC Designation a� — I understand that you are in the process of obtaining your OIC License.
