The UIC Program has reviewed your request and has determined that the information provided to date is sufficient for completing the monitoring and compliance reporting requirements of the permit. Therefore, the permit has been closed. Please note that any other compliance monitoring or reporting for any other regulatory agency shall continue as required. If you wish to conduct future injection operations, you must first apply for and receive a new injection well permit. The injection well permit application, well construction/abandonment forms, and other UIC-related forms can be downloaded from the UIC Program's website at (https://www.deg.nc.gov/about/divisions/water-resources/groundwater- resources/forms-permit-application-tracking). Operating an injection well without a valid permit could result in a civil penalty of up to $25,000 per day. The attached letter is in response to your 12 September 2024 email notice of expiration regarding operating permit number W10800219, for Marine Corps Base Camp Lejeune Resource Conservation and Recovery Act Solid Waste Management Unit (SWMU) 350 Site located southeast of the intersection of Iwo Jima Blvd and State Highway 24, issued on 28 March 2013 by the Underground Injection Control (UIC) Program of the North Carolina Division of Water Resources. Permit No. W10800219 was submitted in 2013 for the operation of biosparging at six vertical air injection wells (IW55-40, IW56-40, IW57-25, IW57-40, IW58-25, IW58-40). As requested, the findings and monitoring data are provided in enclosure (1). No future injections are planned at these wells and a permit rescission request form is provided as enclosure (2). Subsequent Notifications of Intent were submitted to the UIC section to install and operate biosparging injections at Site SWMU 350 and are no longer needed or active. On 7 May 2015, a notice was submitted to add four injection wells (IW59-45, IW60- 45, IW61-45, IW62-45). On 17 May 2016 a notice was submitted to added six injection wells (IW64-60, IW65-40, IW66-40, IW67-40, IW68-40, IW69-40). An additional two injection wells (IW63-45 and IW70-45) were also added over the course of investigations but not included within any intent letters. No future injections are planned at these wells, and findings are presented in enclosures (3 and 4). We request rescission of permits established for these two additional notices at Site SWMU 350. In total, eighteen injections wells were established at Site SWMU 350. Abandonment records of sixteen injection wells are provided as enclosure (5). Two injection wells (IW59-45, IW66-40) were unable to be located at the time of abandonment. Please let us know if you have any additional questions.
